我国城市社区医养结合服务体系构建的理论探究

摘　　要：社会化养老服务重点在基层社区,方向是医养结合。从供给对象、供给内容、供给主体、供给方式和筹资机制五大层面探讨如何构建社区医养结合服务供给模式,提出城乡融合发展视域下的社区医养结合服务应以“轻、中度的失能、失智老人”为主要服务对象,以“生活护理+康复护理”为主要服务内容,构建多方参与、多主体协同的医养服务供给格局;明确政府、社会和市场三大主体的角色定位及责任边界,以组织优化为载体,改变当前以行政化供给为主、社会化供给为补充的供给方式;建立和完善包含社会保险、商业保险、社会福利、个体付费等合理稳定的长效筹资机制。Socialized service for the elderly focuses on the community and directs to the combination of elderly care with medical service.This study discusses the construction of the combination of elderly care with medical service from five aspects,namely,object of supply,content of supply,main body of supply,mode of supply,and the financing mechanism.This paper puts forward that the main supply object should be the elderly with mild,moderate disability and dementia,and the main supply content should be life nursing together with rehabilitation nursing,so as to build a multi-participation,multi-subject coordination of the medical service supply pattern,and clarify the role and responsibility boundary of the government,society and market.We should change the current supply mode of administrative supply-oriented which is supplemented by social supply with organizational optimization as the carrier.Besides,we should build and perfect a reasonable and stable long-term financing mechanism mechanism including social insurance,commercial insurance,social welfare,and individual payment.
