磁共振动脉自旋标记技术诊断非血管性和血管性轻度认知障碍的临床应用价值

摘  要:目的探讨磁共振动脉自旋标记(ASL)技术诊断非血管性和血管性轻度认知障碍的临床应用价值。方法从驻马店市第二人民医院神经内科随机选取23例非血管性轻度认知障碍患者(A组)、23例血管性轻度认知障碍患者(B组)以及23例健康的老年人(对照组),选择时间为2019年9月至2021年9月,运用ASL技术对上述3组人员的认知功能脑区域的脑血流量进行测量,探讨ASL技术对非血管性轻度认知障碍和血管性轻度认知障碍的临床诊断价值。结果相较于对照组,A组双侧楔前叶、扣带回等的脑血流灌注下降,双侧额叶、双侧海马等的脑血流灌注升高(P<0.05);相较于对照组,B组双侧的楔前叶、丘脑等的脑血流灌注下降,双侧额叶、扣带回的脑血流灌注升高(P<0.05);相较于A组,B组右侧颞叶、双侧放射冠的脑血流灌注降低,左侧颞叶、双侧扣带回升高(P<0.05)。结论非血管性和血管性轻度认知障碍脑功能区域脑血流灌注特点不同,通过磁共振动脉自旋标记技术可有效鉴别两者差异,为临床早期诊断提供了有效的数据支持,值得推广。Objective To explore the clinical value of magnetic resonance arterial spin labeling(ASL)in the diagnosis of nonvascular and vascular mild cognitive impairment.Methods A total of 23 patients with nonvascular mild cognitive impairment(group A),23 patients with vascular mild cognitive impairment(group B)and 23 healthy elderly(control group)were randomly selected from the Department of Neurology of Zhumadian Second People’s Hospital from September 2019 to September 2021.The cerebral blood flow in the brain region of cognitive function of the above three groups was measured by ASL technology.The clinical diagnostic value of ASL technology in mild cognitive impairment and vascular mild cognitive impairment were explored.Results Compared with the control group,the cerebral blood flow perfusion of bilateral precuneus and cingulate gyrus decreased,and the cerebral blood flow perfusion of bilateral frontal lobe and bilateral hippocampus increased in group A(P<0.05).Compared with the control group,the cerebral blood flow perfusion of bilateral precuneus and thalamus decreased,and the cerebral blood flow perfusion of bilateral frontal lobe and cingulate gyrus increased in group B(P<0.05).Compared with group A,the cerebral blood flow perfusion of the right temporal lobe and bilateral radial crowns decreased,and the left temporal lobe and bilateral cingulate gyrus increased in group B(P<0.05).Conclusion Nonvascular and vascular mild cognitive impairment have different characteristics of cerebral blood flow perfusion in brain functional areas.Magnetic resonance arterial spin labeling can effectively identify the differences between them,which provides effective data support for early clinical diagnosis and is worthy of popularization.
