摘　　要：目的探讨风湿病并发带状疱疹(herpes zoster,HZ)患者淋巴细胞亚群和细胞因子的变化特征。方法回顾性选择2020年1月—2022年1月解放军总医院第七医学中心收治的144例风湿病患者的临床资料,将其中发生HZ的48例设为观察组,未出现HZ的96例作为对照组,比较两组患者的淋巴细胞亚群和细胞因子水平。结果与对照组相比,观察组总T细胞、NK细胞、CD3^(+)CD4^(+)细胞、CD4^(+)/CD8^(+)比值、Treg细胞明显减少,CD3^(+)CD8^(+)细胞明显增多,差异有统计学意义(P<0.05);两组B细胞水平比较,差异无统计学意义(P>0.05)。与对照组相比,观察组白细胞介素-6和肿瘤坏死因子-α明显增多,白细胞介素-10明显减少,差异有统计学意义(P<0.05);两组白细胞介素-2、白细胞介素-4和干扰素γ比较,差异无统计学意义(P>0.05)。结论风湿病患者并发HZ感染时总T细胞、NK细胞、CD3^(+)CD4^(+)细胞、CD4^(+)/CD8^(+)比值、Treg细胞明显减少、CD3^(+)CD8^(+)细胞增多,同时白细胞介素-6、肿瘤坏死因子-α增多,白细胞介素-6减少。Objective To explore the changes of lymphocyte subsets and cytokines in patients with rheumatism com⁃plicated by herpes zoster(HZ).Methods The clinical data of 144 rheumatic patients admitted to the Seventh Medical Center of the General Hospital of the People's Liberation Army from January 2020 to January 2022 were retrospec⁃tively selected.Among them,48 cases with HZ were set as the observation group,and 96 cases without HZ were set as the control group.The lymphocyte subsets and cytokine levels of the two groups were compared.Results Com⁃pared with the control group,the total T cells,NK cells,CD3^(+)CD4^(+)cells,CD4^(+)/CD8^(+)ratio and Treg cells in the obser⁃vation group were significantly decreased,and CD3^(+)CD8^(+)cells were significantly increased,the difference was statisti⁃cally significant(P<0.05).There was no statistically significant difference in B cell level between the two groups(P>0.05).Compared with control group,interleukin-6 and tumor necrosis factor-αwere significantly increased and inter⁃leukin-10 was significantly decreased in observation group,the difference was statistically significant(P<0.05).There was no statistically significant difference in interleukin-2,interleukin-4 and tumor necrosis factor-γbetween the two groups(P>0.05).Conclusion When patients with rheumatism complicated with HZ infection,the total T cells,NK cells,CD3^(+)CD4^(+)cells,CD4^(+)/CD8^(+)ratio,Treg cells decreased significantly,CD3^(+)CD8^(+)cells increased,and interleu⁃kin-6,tumor necrosis factor-αincreased,interleukin-10 reduced.
