摘 要:目的分析血栓弹力图联合血小板抗体检测对心脏手术围术期输血的指导作用。方法选取2021年10月—2023年2月徐州市肿瘤医院接受心脏手术围术期输血治疗的67例患者为研究对象,按照输血方法分组,以血栓弹力图检测指导输血的33例为对照组,以血栓弹力图联合血小板抗体检测指导输血的34例为观察组。比较两组成分血输注量、校正指数及不良反应发生情况。结果观察组成分血输注量低于对照组,差异有统计学意义(P<0.05)。观察组输血后1、24 h时的校正指数均高于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率为5.88%较对照组的27.27%更低,差异有统计学意义(χ^(2)=5.584,P<0.05)。结论将血栓弹力图联合血小板抗体检测用于心脏手术围术期输血指导中能够有效减少各成分血输注量,并提升校正指数,减少输血相关并发症。Objective To analyze the guiding effect of thrombelastography combined with platelet antibody detection on perioperative blood transfusion in cardiac surgery.Methods 67 patients who underwent perioperative blood transfu⁃sion treatment for cardiac surgery at Xuzhou Cancer Hospital from October 2021 to February 2023 were selected as the study subjects.They were grouped according to blood transfusion methods,with 33 patients guided by thromboelas⁃tography as the control group and 34 patients guided by thromboelastography combined with platelet antibody testing as the observation group.Compared the component blood transfusion volume,correction index,and incidence of ad⁃verse reactions between the two groups.Results The observation group component blood transfusion volume was lower than the control group,and the difference was statistically significant(P<0.05).The correction index of the observation group at 1 hours and 24 hours after blood transfusion was higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 5.88%,which was lower than the control group's 27.27%,and the difference was statistically significant(χ^(2)=5.584,P<0.05).Conclusion The use of thrombelastography combined with platelet antibody detection in perioperative transfusion guidance for car⁃diac surgery can effectively reduce the amount of each component blood transfusion and improve the corrected count increment to reduce transfusion-related complications.
