支撑喉镜下针状电刀喉肿瘤切除术治疗早期喉癌与喉癌前病变的疗效以及安全性分析

摘　　要：目的探讨支撑喉镜下针状电刀喉肿瘤切除术治疗早期喉癌与喉癌前病变的疗效以及安全性。方法选取2020年6月—2022年6月在高州市人民医院接受支撑喉镜下针状电刀喉肿瘤切除术治疗的120例早期喉癌与喉癌前病变患者,以数字随机表法分为两组,各60例。对照组采用传统开放手术切除术,研究组采用喉镜下针状电刀切除术,对比两组的术后并发症、术后复发率以及声带修复情况变化。结果与对照组相比,研究组治疗后的气道灼烧、吞咽不适、声带粘连和呼吸困难等术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。与对照组相比,研究组在治疗后复发率的患者比例(0)低于对照组(10.00%),差异有统计学意义(χ^(2)=4.386,P<0.05)。对照组患者声带基本恢复率低于研究组,瘢痕或缺损率高于研究组,差异有统计学意义(P<0.05)。结论在早期喉癌与喉癌前病变的治疗过程中,支撑喉镜下针状电刀喉肿瘤切除术疗效显著,有一定的优越性和发展力,是一种高效的治疗方法。Objective To investigate the curative effect and safety of needle electrocautery laryngeal tumor resection under support laryngoscope in the treatment of early laryngeal cancer and precancerous lesions.Methods A total of 120 patients with early laryngeal cancer and precancerous lesions who underwent needle shaped electrosurgical resec⁃tion of laryngeal tumors under supportive laryngoscope at the People's Hospital of Gaozhou City from June 2020 to June 2022 were selected and divided into two groups using a digital random table method,with 60 patients in each group.The control group underwent traditional open surgical resection,while the study group underwent needle shaped electric knife resection under laryngoscope.The postoperative complications,recurrence rate,and changes in vocal cord repair were compared between the two groups.Results Compared with the control group,the incidence of postoperative complications such as airway burning,swallowing discomfort,vocal cord adhesion,and dyspnea in the study group after treatment were lower than those in the control group,the difference was statistically significant(P<0.05).Compared with the control group,the proportion of patients with recurrence rate after treatment in the study group(0)was lower than that in the control group(10.00%),the difference was statistically significant(χ^(2)=4.386,P<0.05).The basic recovery rate of vocal cords in the control group was lower than that in the study group,and the scar or defect rate was higher than that in the study group,the difference was statistically significant(P<0.05).Conclusion In the treatment of early laryngeal cancer and precancerous lesions of the larynx,needle electrocautery laryngeal tu⁃mor resection under the support laryngoscope is effective.It has certain advantages and development potential,is an efficient treatment method.
