负压吸引鞘与普通软镜鞘在输尿管软镜碎石术中的效果

摘　　要：目的探讨负压吸引鞘与普通软镜鞘在输尿管软镜碎石术中的应用效果。方法选取2021年1月—2022年12月睢宁县人民医院收治的行输尿管软镜碎石术患者46例,按随机数表法分为两组,23例/组。对照组采用普通软镜鞘联合输尿管软镜碎石术,观察组采用负压吸引鞘联合输尿管软镜碎石术。对比两组围术期情况、肾盂内压、感染状况、疼痛程度和舒适度及并发症。结果观察组手术时间、住院时间为(49.65±3.21)min、(3.62±0.28)d,短于对照组的(52.34±4.28)min、(5.13±0.89)d,术中出血量为(3.45±0.78)mL,少于对照组的(10.67±2.64)mL,差异有统计学意义(t=2.411、9.094、7.762,P<0.05);观察组肾盂内压最小值、最大值及平均值均小于对照组,差异有统计学意义(P<0.05);观察组感染状况均比对照组更低,差异有统计学意义(P<0.05);术后8、36 h,观察组疼痛程度评分低于对照组,舒适度评分高于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论负压吸引鞘应用于输尿管软镜碎石术中可改善围术期情况及舒适度,降低肾盂内压,减轻感染及疼痛状况,安全性高,值得推广。Objective To explore the application effect of negative pressure suction sheath and ordinary flexible mir⁃ror sheath in flexible ureteroscopic lithotripsy.Methods The data of 46 patients who underwent flexible uretero⁃scopic lithotripsy in Suining County People's Hospital from January 2021 to December 2022 were selected.Accord⁃ing to the random number table method,they were divided into two groups,with 23 cases in each group.The control group received ordinary soft mirror sheath combined with flexible ureteroscope lithotripsy,and the observation group received negative pressure suction sheath combined with flexible ureteroscope lithotripsy.The perioperative condi⁃tions,renal pelvis pressure,infection status,pain degree,comfort and complications were compared between the two groups.Results The operation time and hospital stay of the observation group were(49.65±3.21)min and(3.62±0.28)d,which were shorter than those of the control group(52.34±4.28)min and(5.13±0.89)d,and the intraoperative blood loss was(3.45±0.78)mL,which was less than that of the control group(10.67±2.64)mL,and the difference was statis⁃tically significant(t=2.411,9.094,7.762,P<0.05).The minimum,maximum and average values of intrapelvic pressure in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The infection status of observation group was lower than that of control group,the difference was statistically significant(P<0.05).At 8 and 36 h after surgery,the pain degree score of the observation group was lower than that of the control group,and the comfort score was higher than that of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The application of negative pressure suction sheath in flexible ureteroscopic lithotripsy can im⁃prove perioperative conditions and comfort,reduce renal pelvis pressure,and reduce infection
