摘　　要：目的以超声心动图(Echo)为标准,探讨320排CT冠脉成像(CCTA)评价老年冠心病患者左心室功能的诊断价值。方法回顾性分析2021年7月至2022年5月收治的78例冠心病患者的临床影像资料,所有患者在1周内接受CCTA与Echo检查,运用心功能分析软件分别计算出射血分数(EF)、每搏输出量(SV)、左心室舒张末期容积(EDV)和收缩末期容积(ESV),以Echo左心室功能的评估结果为标准,采用配对样本T检验以及Pearson相关分析比较两种检验方法的差异和一致性。结果2种检查方式对于冠心病患者左心室功能的评价具有较好的一致性,320-CCTA和Echo左心室功能指标相比,EF、SV、EDV和ESV之间均无显著差异(P>0.05);通过组内相关系数(ICC)分析显示:CCTA与Echo左心室功能测量结果一致性较好(P<0.05)。结论以Echo为标准,CCTA在诊断冠心病患者左心室功能方面结果准确,可以一站式完成冠状动脉狭窄程度与左心室功能的评估,具有较好的临床应用前景。Objective To explore the diagnostic value of the 320-row coronary computed tomography angiography(CCTA)in assessing left ventricular function in elderly patients with coronary heart disease with the echocardiographic(Echo)findings as the gold standard.Methods Clinical imaging data of 78 patients with coronary heart disease treated from July 2021 to May 2022 were retrospectively analyzed.All patients were examined by CCTA and Echo within a week.The ejection fraction(EF),stroke volume(SV),left ventricular end-diastolic volume(EDV),and left ventricular end-systolic volume(ESV)were recorded using the cardiac function analysis software.With Echo findings as the gold standard,the differences and consistency of left ventricular function assessed by CCTA and Echo were examined by the pairing sample t test,and Pearson correlation analysis.Results Both CCTA and Echo effectively had a good consistency on assessed the left ventricular function of coronary heart disease patients.There were no significant differences in EF,SV,EDV,and ESV examined by CCTA and Echo(P>0.05).Analysis of the intra-class correlation coefficient(ICC)showed that CCTA CCTA had a good consistency on assessing the left ventricular function of coronary heart disease patients with Echo(P<0.05).Conclusion With Echo findings as gold standard,CCTA accurately assesses the left ventricular function of coronary heart disease patients.CCTA provides one-stop assessment of coronary artery stenosis and left ventricular function,showing an acceptable clinical application prospect.
