摘 要:血栓性疾病是临床多发病,抗血小板药物是预防及治疗血栓性疾病的重要手段。随着抗血小板药的广泛应用,用药过量导致患者出血事件的发生率逐年增高。通过血小板功能检测来评估用药效果、指导临床用药,可明显降低临床出血不良事件的发生率。连续计数法血小板功能检测是近年来开展的一项评价血小板聚集功能的新技术,无需离心,也无需提取贫、富血小板血浆等手工操作,可实现全血标本、全流程自动化检测需求。与其他传统血小板功能检测实验相比,具有操作简单、干扰因素少、自动化程度高、真正模拟体内全血环境等优势;但也有不足,如红细胞碎片影响血小板计数,采用固定时间点检测血小板数量,而非连续线性监测影响实验稳定性等。本综述将连续计数法与其他血小板功能检测方法进行比较,分析各自优势与不足,并对检测过程中的影响因素进行具体分析,提出优化方案与质量改进措施。Thrombotic disease is a popular disease,and antiplatelet drugs are important agents to prevent and treat it.With the widespread use of antiplatelet drugs,the incidence of bleeding events due to overdose has annually increased.Detection of platelet function to assess the drug effect and guide clinical medication can significantly reduce the incidence of bleeding-related adverse events.Sequential platelet counting method(SPCM)is an emerging technology to assess the platelet aggregation function,which is independent of centrifugation,and extraction of platelet-poor and platelet-rich plasma.Moreover,it can be used to automatically detect whole blood samples.Compared with other conventional testing for platelet function,SPCM is featured by simple procedures,less interference factors,high degree of automation,and mimic of the whole blood environment.It also has shortcomings.For example,red blood cell fragments may affect the platelet count;the detection time point is fixed;and non-continuous linear monitoring affects experimental stability.This review compares advantages and disadvantages of SPCM and other conventional testing for platelet function,and specifically analyzes the influencing factors in the detection process.We also propose optimization schemes and quality improvement measures.
