摘　　要：目的:观察经食管超声心动图检查(TEE)评估左心耳封堵术中塞式、盘式封堵器的价值。方法:选取2017年11月~2023年1月在本院拟行左心耳封堵术的非瓣膜性房颤患者共100例进入研究。收集所有患者的TEE资料,分析TEE在塞式、盘式左心耳封堵器术中的监测要点。结果:95例患者成功植入封堵器,成功率95.0%(95/100)。其中,47例采用塞式封堵器,48例用盘式。两组封堵器中TEE测量的左心耳开口最大直径与所选用封堵器平均尺寸均密切相关(P<0.001)。释放伞即刻塞式封堵器10例出现残余分流,盘式12例,两种器械残余分流发生率差异无统计学意义(χ^(2)=0.185,P=0.667)。但塞式封堵器残余分流颈宽度小于盘式封堵器例(t=3.120,P=0.01)。结论:TEE评估不同类型封堵器有不同侧重点,精准影像评估对于左心耳封堵术具有重要指导意义。Objective:To evaluate the value of transesophageal echocardiography(TEE)in two different types of occluders in left atrial appendage closure(LAAO).Methods:Selected a total of 100 non-valvular atrial fibrillation(NVAF)patients who planned to undergo left atrial appendage occlusion in our hospital from November 2017 to January 2023.TEE data of all patients were collected,and analyze the monitoring points of TEE in the peri-operative period of different left atrial appendage occluder.Results:95 patients were successfully implanted with occluders,with a success rate of 95.0%(95/100).Among them,47 cases were treated with Watchman occluder and 48 cases were treated with lobe-and-disc occluder.The maximum diameter of the left atrial appendage opening measured by TEE was closely related to the average size of the selected occluder(P<0.001).Moreover,residual shunt occurred in 10 cases with Watchman occluder and 12 cases with disk occluder.There was no significant difference in the incidence of residual shunt between the two devices(χ^(2)=0.185,P=0.667).However,the residual shunt neck width of the Watchman occluder was smaller than that of the lobe-and-disc occluder(t=3.120,P=0.01).Conclusion:Different types of occluders have different focuses in LAAO when evaluating by TEE.Accurate imaging evaluation is of great guiding significance for LAAO.
