尺动脉腕上皮支下行支皮支血管的应用解剖

摘　　要：目的 为尺动脉腕上皮支下行支皮支皮瓣的临床应用提供解剖数据。方法 选取10侧新鲜上肢标本进行解剖研究,其中5侧标本乳胶灌注血管进行显微解剖,5侧制作铸型标本,拍照记录尺动脉腕上皮支下行支皮支的起始、走行和吻合,测量皮支的直径和蒂长。结果 尺动脉腕上皮支下行支平均发出7.3支皮支,对在近、远侧发出的皮支数进行比较发现差异有统计学意义(P<0.05);尺动脉腕上皮支下行支在近、远侧发出皮支的直径和蒂长统计分析发现差异也无统计学意义(P>0.05)。尺动脉腕上皮支下行支发出的皮支吻合形成血管链供应皮瓣,同时与尺神经手背支伴行。结论 尺动脉腕上皮支下行支皮支皮瓣能以近侧或远侧皮支为血管蒂可能就近修复手腕部皮肤缺损。Objective To provide anatomical data for the clinical application of the cutaneous branch flap of the descending branch of the carpal epithelial branch of the ulnar artery.Methods Ten fresh upper limb speci⁃mens were selected for anatomical study,five of which were micro⁃anatomized with latex perfusion vessels,and five were made with cast specimens.Photographs were taken to record the origin,course and anastomosis of the cutane⁃ous branch of the descending branch of the carpal epithelial branch of the ulnar artery,and the diameter and pedicle length of the cutaneous branch were measured.Results The descending branch of the carpal epithelial branch of the ulnar artery sent out an average of 7.3 cutaneous branches.There was no statistical difference in the number of cu⁃taneous branches sent out in the proximal and distal sides(P<0.05).Statistical analysis showed that there was no sig⁃nificant difference in the diameter and pedicle length of the proximal and distal cutaneous branches of the descending branch of the carpal epithelial branch of the ulnar artery(P>0.05).The cutaneous branch from the descending branch of the carpal epithelial branch of the ulnar artery anastomoses to form a vascular chain to supply the skin flap,and was accompanied by the dorsal branch of the ulnar artery.Conclusion The cutaneous branch flap of the descending branch of the carpal epithelial branch of the ulnar artery can repair the skin defect of the wrist with the proximal or distal cutaneous branch as the vascular pedicle.
