老年大腿部带状疱疹1例

摘　　要：带状疱疹是由水痘带状疱疹病毒感染所导致的一种常见皮肤疾病,其发病机制主要与机体免疫力下降,激活潜伏于神经细胞内的病毒有关。本疾病主要以神经节分布区域出现簇集疱疹样皮损和神经疼痛为主要表现。老年人群是一类特殊人群,常常因高龄、基础疾病多、营养状况差等综合因素,致其细胞和体液免疫功能降低,因而成为带状疱疹好发的一类主要人群。大部分老年带状疱疹患者早期主要表现为胸部、背部或腰腹部皮肤出现红斑或丘疹,同时在此基础上逐渐出现大片簇集状水泡。但对于老年带状疱疹患者,常常由于合并其他多器官功能损害、起病隐匿等原因,可能导致临床症状不典型,从而更容易造成疾病的误诊或漏诊,进而影响疾病的及时诊治。本文通过介绍中国人民解放军第九五八医院全科医学科近期收治的1例大腿部带状疱疹(发病部位较为少见)病例诊治过程,旨在进一步认识老年带状疱疹可能存在的不典型性和特殊性,从而为年轻临床医师在今后老年带状疱疹的相关临床诊治工作中提供经验与借鉴。Herpes zoster,a common skin disease,is caused by varicella zoster virus infection,and its pathogenesis is mainly related to the decline of immunity and the activation of viruses latent in nerve cells.The disease is mainly characterized by clustered herpetiform lesions and nerve pain in ganglionic distribution areas.As a special population,The elderly often have,due to advanced age,underlying diseases,poor nutritional status,and other comprehensive factors,resulting in reduced cel-lular and humoral immune function,and thus become a major population with a predilection for herpes zoster.Most elderly patients with herpes zoster mainly featured with erythema or papules on the skin of the chest,back,or waist and abdomen in the early stage,while large clusters of blisters gradually appear on this basis.However,elderly patients with herpes zos-ter,often due to other combined multiple organ function damage,insidious onset,and other reasons,may present atypical clinical symptoms,which is more likely to result in misdiagnosis or missed diagnosis of the disease.This affects the timely diagnosis and treatment of the disease.In this paper,a case of herpes zoster of the thigh(rare site of disease)recently ad-mitted to the Department of General Practice of the 958th Hospital is introduced in order to further understand the possible atypia and particularity of elderly patients with herpes zoster to provide experience and evidence for young clinicians in its fu-ture relevant clinical diagnosis and treatment in this population.
