高频超声在肩关节及周围软组织病变诊断中的临床应用价值

摘　　要：目的:分析在肩关节及周围软组织病变诊断中应用高频超声的临床价值。方法:采集本院2021年1月~2022年12月收治的78例疑似肩关节及周围软组织病变患者为研究对象,入院患者均行高频超声检查,对肩关节及周围软组织病变的影像学特征进行分析。结果:本次研究纳入78例患者经手术验证为肩关节及周围软组织病变(阳性)75例,非肩关节及周围软组织病变(阴性)3例;高频超声检查阳性73例,阴性5例;对比手术结果,高频超声检查检出率为92.31%、检查灵敏度为96.00%、检查特异度为66.67%、漏诊率为4.00%。高频超声检查与手术结果在肩袖撕裂、肱二头肌长头腱撕裂、肩关节周围滑囊积液、钙化性肌腱炎、肌腱增厚、肌腱撕裂检出率方面对比无差异(P>0.05)。结论:在肩关节及周围软组织病变诊断中应用高频超声检查准确性、灵敏度均较高,在具体病症定性诊断中也具有较高准确率,可为病情评估及预后治疗提供科学参考。Objective:To analyze the clinical value of high-frequency ultrasound in the diagnosis of shoulder joint and surrounding soft tissue lesions.Methods:78 patients with suspected shoulder joint and surrounding soft tissue lesions admitted to our hospital from January 2021 to December 2022 were collected as the research subjects.All admitted patients underwent high-frequency ultrasound examination to analyze the imaging characteristics of shoulder joint and surrounding soft tissue lesions.Results:78 patients were included in this study,and 75 patients were surgically confirmed as having shoulder joint and surrounding soft tissue lesions(positive),while 3 patients were non shoulder joint and surrounding soft tissue lesions(negative);73 cases were positive and 5 cases were negative in high-frequency ultrasound examination;Compared with the surgical results,the detection rate of high-frequency ultrasound was 92.31%,the sensitivity was 96.00%,the specificity was 66.67%,and the missed diagnosis rate was 4.00%.There was no difference between high-frequency ultrasound examination and surgical results in the detection rate of rotator cuff tear,long head tendon tear of biceps brachii,synovial effusion around the shoulder joint,calcified tendinitis,tendon thickening,and tendon tear(P>0.05).Conclusion:The application of high-frequency ultrasound in the diagnosis of shoulder joint and surrounding soft tissue lesions has high accuracy and sensitivity,and also has high accuracy in the qualitative diagnosis of specific diseases.It can provide scientific reference for disease assessment and prognosis treatment.
