摘  要:目的:比较单光子发射计算机断层摄影术联合同机CT扫描图像融合技术(SPECT-CT)与磁共振成像(MRI)在骨质疏松性椎体压缩性骨折(OVCF)疼痛责任椎体检查的效果。方法:回顾性分析2020年5月~2021年5月本院收治的74例(145个椎体)OVCF患者病案相关资料,其中53例为急性期骨折(病程≤3周),21例为亚急性期骨折(病程4~12周)。对所有患者施行SPECT-CT与MRI检查,记录疼痛责任椎体检出情况,并对其施行经皮椎体后凸成形术(PKP),于术前、术后3d以视觉模拟疼痛量表(VAS)评估疼痛度。结果:53例急性期OVCF患者共86个椎体施行PKP术,其中两种检查方式共同确诊82个疼痛责任椎体,Kappa值为0.553,一致性较为理想;而21例亚急性期OVCE患者共38个椎体行PKP术治疗,其中两种检查方式共同确诊33个疼痛责任椎体,Kappa值为0.414,一致性较为理想。术后3d,所有OVCF患者VAS评分较术前低,有统计学差异(P<0.05)。结论:SPECT-CT、MRI两者可有效诊断OVCF疼痛责任椎体,一致性较为理想,且对难以行MRI检查者,可选用SPECT-CT进行诊断。Objective:To compare single photon emission computed tomography combined with CT scan image fusion technology(SPECT-CT)and magnetic resonance imaging(MRI)in osteoporotic vertebral compression fracture(OVCF)pain-related vertebral examination internal effect.Methods:The medical records of 74 patients(145 vertebral bodies)with OVCF admitted to our hospital from May 2020 to May 2021 were retrospectively analyzed,of which 53 were acute fractures(duration≤3 weeks)and 21 were subacute stage fracture(duration 4-12 weeks).SPECT-CT and MRI were performed on all patients to record the findings of the vertebral body responsible for pain,and percutaneous kyphoplasty(PKP)was performed on them.The visual analog pain scale(VAS)to assess pain.Results:A total of 86 vertebral bodies in 53 patients with acute OVCF underwent PKP.Among them,82 vertebral bodies with pain responsibility were diagnosed by the two examination methods,and the Kappa value was 0.553,which was an ideal agreement.In 21 patients with subacute OVCE,a total of 38 All vertebral bodies were treated with PKP,and 33 pain-responsible vertebral bodies were diagnosed by the two examination methods.The Kappa value was 0.414,and the consistency was ideal.On the 3rd day after operation,the VAS scores of all OVCF patients were lower than those before operation,and there was a statistical difference(P<0.05).Conclusion:Both SPECT-CT and MRI can effectively diagnose the vertebral body responsible for pain in OVCF,and the consistency is ideal.For those who are difficult to perform MRI examination,SPECT-CT can be used for diagnosis.
