Value of four dimensional dynamic cardiac volume imaging combined with transthoracic echocardiograph in diagnosis of complex congenital heart disease

摘  要:目的 探讨心脏四维容积动态成像(4D-CCTA)联合经胸超声心动图(TTE)诊断复杂先心病的价值。方法 选取周口市中医院2021年2月至2023年1月收治的接受房间隔缺损(ASD)、室间隔缺损(VSD)修补术患者79例作为研究对象,均行4D-CCTA、TTE检查,以手术结果作为金标准,分析4D-CCTA联合TTE的诊断价值。结果 手术结果共发现病变142处;TTE联合4D-CCTA诊断准确率[95.07%(135/142)]均明显高于单一4D-CCTA检测[75.35%(107/142)]、TTE检测[61.27%(87/142)],且联合检测Kappa指数[0.768(95%CI:0.643~0.859)]均高于单一4D-CCTA检测[0.710(95%CI:0.548~0.759)]、TTE检测[0.649(95%CI:0.552~724),P<0.05]。结论 4D-CCTA联合TTE应用于复杂先心病中,可提高诊断准确率,为临床治疗方案完善提供依据。Objective To investigate the value of four dimensional dynamic cardiac volume imaging(4D-CCTA) combined with transthoracic echocardiography(TTE) in diagnosing complex congenital heart disease.Methods Seventy-nine patients with atrial septal defect(ASD) and ventricular septal defect(VSD) repairment in Zhoukou Hospital of Traditional Chinese Medicine from February 2021 to January 2023 were selected as the research objects.All patients underwent 4D-CCTA and TTE examinations.The surgical results were used as the gold standard to analyze the diagnostic value of 4D-CCTA combined with TTE.Results A total of 142 lesions were found.The diagnostic accuracy of TTE combined with 4D-CCTA [95.07%(135/142)] was significantly higher than that of single 4D-CCTA test [75.35%(107/142)] and TTE test [61.27%(87/142)],and Kappa index [0.768(95%CI:0.643~0.859)] were higher than single 4D-CCTA detection [0.710(95%CI:0.548~0.759)] and TTE detection [0.649(95%CI:0.552~724,P<0.05)].Conclusions 4D-CCTA combined with TTE can improve the diagnostic accuracy in complex congenital heart disease and provide evidence for the improvement of clinical treatment.
