摘　　要：目的:探讨两种不同敷料联合自体微粒皮种植术对深Ⅱ度烧伤的疗效。方法:选取2018年9月-2020年10月在笔者医院就诊的90例深Ⅱ度烧伤患者,按随机数表法分为对照组和试验组,各45例。对照组给予湿润烧伤膏联合自体微粒皮种植术进行治疗;试验组给予水凝胶敷料联合自体微粒皮种植术进行治疗。比较两组患者疗效、创面愈合情况、温哥华瘢痕量表(Vancouver scar scale,VSS)评分、血清致痛因子水平及不良反应。结果:治疗3周后,两组患者总有效率比较差异无统计学意义(P>0.05);试验组患者创面上皮形成时间、创面愈合时间及不良反应发生率数值均低于对照组(P<0.05);治疗3周后,两组血清前列腺素E2(Prostaglandin,PGE2)、神经肽(Neuropetide Y,NPY)、5-羟色胺(5-Hydroxytryptamine,5-HT)水平均降低,且试验组低于对照组(P<0.05)。治疗2个月后,试验组VSS评分低于对照组(P<0.05)。结论:与湿润烧伤膏联合自体微粒皮种植术治疗深Ⅱ度烧伤效果相比,水凝胶敷料联合自体微粒皮种植术治疗深Ⅱ度烧伤可提升创面愈合速度,降低血清致痛因子水平,减少瘢痕形成,且不良反应较少,值得临床应用。Objective To explore the curative effect of two different dressings combined with autologous micro-skin grafting on deep degree-II burn.Methods 90 patients with deep degree-II burn treated in the author's hospital from September 2018 to October 2020 were selected and divided into control group and experimental group according to the random number table method,with 45 cases in each group.The control group was treated with moist burn ointment combined with autologous microskin grafting.The experimental group was treated with hydrogel dressing combined with autologous microskin grafting.The curative effect,wound healing,vancouver scar scale(VSS)score,serum pain factor levels,and adverse reactions were compared between the two groups.Results After 3 weeks of treatment,there was no significant difference in the total effective rate between the two groups(P>0.05).The wound epithelial formation time,wound healing time,and incidence of adverse reactions in the experimental group were lower than those in the control group(P<0.05).After 3 weeks of treatment,the serum levels of Prostaglandin E2(PGE2),Neuropetide Y(NPY)and 5-Hydroxytryptamine(5-HT)in the two groups were decreased,and those in the experimental group were lower than those in the control group(P<0.05).After 2 months of treatment,the VSS score of the experimental group was lower than that of the control group(P<0.05).Conclusion Compared with moist burn ointment combined with autologous microskin grafting in the treatment of deep partial-thickness burn,hydrogel dressing combined with autologous microskin grafting can improve wound healing speed,reduce serum levels of pain factors,reduce scar formation,and has fewer adverse reactions,which is worthy of clinical application.
