创伤性肾出血使用弹簧圈联合明胶海绵颗粒肾动脉栓塞术治疗的临床观察

摘　　要：目的观察医用弹簧圈联合明胶海绵颗粒进行肾动脉栓塞术用于治疗创伤性肾出血的临床效果。方法以130例创伤性肾出血患者为研究对象,按治疗方式不同分为研究组(65例)和对照组(65例),对照组行明胶海绵颗粒肾动脉栓塞术治疗,研究组行医用弹簧圈联合明胶海绵颗粒肾动脉栓塞术治疗。记录患者手术指标,检测手术前后血清炎症因子水平及肾功能指标变化,观察术后并发症及复发性出血情况。结果两组手术时长、血尿消失时间、住院时间和院内输血量差异均不具有统计学意义(P>0.05);研究组术后15 d血液SCr、BUN、UA和Cys C含量均显著低于对照组水平(P<0.05);研究组术后15 d血清TNF-α、IL-1β和IL-6含量均显著低于对照组水平(P<0.05);研究组术后1个月内并发症总体发生率与对照组水平差异不具有统计学意义(P<0.05);而研究组复发性出血发生率显著低于对照组水平(P<0.05)。结论医用弹簧圈联合明胶海绵颗粒肾动脉栓塞术治疗创伤性肾出血,可有效缓解肾功能损伤,降低术后炎症反应程度,提高肾出血栓塞效果稳定性和持久性。Objective To observe the clinical effect of arterial spring coil combined with gelatin sponge particle renal artery embolization in the treatment of traumatic renal hemorrhage.Methods 130 patients with traumatic renal hemorrhage were divided into study group(65 cases)and control group(65 cases).The patients in he control group were treated with renal artery embolization of gelatin sponge particles,and in the study group were treated with arterial spring coil combined with renal artery embolization of gelatin sponge particles.The operation indexes,the changes of serum inflammatory factors and renal function indexes before and after operation,and the postoperative complications and recurrent bleeding were recorded.Results There was no significant difference in operation time,hematuria disappearance time,hospital stay and blood transfusion between the two groups(P>0.05).The contents of blood SCR,BUN,UA and Cys C in the study group were significantly lower than those in the control group 15 days after operation(P<0.05).The level of serum TNF-α、IL-1βand IL-6 in the study group 15 days after operation were significantly lower than those in the control group(P<0.05).There was no significant difference in the overall incidence of complications between the study group and the control group within one month after operation(P<0.05).The incidence of recurrent bleeding in the study group was significantly lower than that in the control group(P<0.05).Conclusion Arterial coil combined with gelatin sponge particle renal artery embolization in the treatment of traumatic renal hemorrhage can effectively alleviate the injury of renal function,reduce the degree of postoperative inflammatory reaction,and improve the stability and durability of embolization effect.
