经皮微创钢板内固定治疗胫骨平台骨折的效果观察

摘　　要：目的:观察经皮微创钢板内固定治疗胫骨平台骨折的效果。方法:选取2020年3月—2022年6月期间于南京医科大学附属苏州医院骨科接受手术治疗的70例胫骨平台骨折患者,依照手术治疗的方式不同分为研究组和参照组各35例。参照组实施切开复位钢板内固定治疗,研究组实施经皮微创钢板内固定治疗,治疗结束后比较两组患者的围手术期指标以及膝关节功能恢复情况。结果:研究组患者的手术时间、术中出血量、手术切口长度以及住院时间均优于参照组患者,差异有统计学意义(P<0.05);术前,两组患者前列腺素E2(PGE2)、神经肽Y(NPY)以及P物质(SP)的水平比较,差异无统计学意义(P>0.05);术后,两组患者的PGE2、NPY以及SP的水平均高于术前,但研究组患者的PGE2、NPY以及SP水平低于参照组,差异有统计学意义(P<0.05)。研究组患者的膝关节功能恢复优良率(94.29%)高于参照组(68.57%),差异有统计学意义(P<0.05)。结论:经皮微创钢板内固定治疗胫骨平台骨折患者效果较好,具有创伤小、住院时间短等多种优势。Objective In this study,percutaneous minimally invasive plate internal fixation was applied in the treatment of tibial plateau fracture,and the clinical efficacy of this operation was analyzed first.Methods 70 patients with tibial plateau fracture who received surgical treatment in the orthopedics department of Suzhou Hospital Affiliated to Nanjing Medical University from March 2020 to June 2022 were included in the study.The subjects were divided into study group(n=35)and reference group(n=35)according to different surgical treatment methods.The reference group was treated with open reduction plate internal fixation,while the study group was treated with percutaneous minimally invasive plate internal fixation.After treatment,the perioperative indexes and knee functional recovery of the two groups were compared.Results The operation time,intraoperative blood loss,surgical incision length and hospital stay in the study group were better than those in the reference group(P<0.05).There were no significant differences in the levels of prostaglandin E2(PGE2),neuropeptide Y(NPY)and substance P(SP)between the two groups before surgery(P>0.05).After surgery,the levels of PGE2,NPY and SP were increased in both groups,and the fluctuation degree of the study group was lower than that of the reference group(P<0.05).The excellent and good rate of knee function recovery in the study group(94.29%)was higher than that in the reference group(68.57%)(P<0.05).Conclusions The treatment of patients with tibial plateau fracture with percutaneous minimally invasive plate internal fixation is effective,and has many advantages,such as less trauma and shorter hospital stay.
