摘　　要：目的分析持续封闭负压引流技术(VSD)联合纳米银医用抗菌敷料对糖尿病足慢性难愈创面疗效和炎性因子的影响。方法选取2020年1月—2022年8月内蒙古自治区第三医院收治的120例糖尿病足慢性难愈创面患者,通过随机数表法分为两组,对照组57例和观察组63例。所有患者入院后给予抗感染、控制血糖和创面彻底清创,对照组清创术后只用VSD覆盖创面,观察组清创后将重组人表皮生长因子凝胶均匀的敷在创面上,再用VSD覆盖创面,待两组患者创面达到移植皮片的条件时给予植皮术。比较两组患者创面愈合率、创面愈合速率、创缘反应及渗出量、炎性因子和疗效指标。结果观察组的创面愈合率、创面愈合速率、炎性因子和疗效指标均显著高于对照组,差异有统计学意义(P<0.05),观察组的创缘反应及渗出量著低于对照组,差异有统计学意义(P<0.05)。结论VSD联合重组人表皮生长因子凝胶能有效促进肉芽组织生长、缩短创面愈合时间和提高创面植皮愈合质量,有效提高糖尿病足慢性难愈合创面植皮存活率。Objective To analyze the effects of vacuum sealing drainage(VSD)combined with Nano silver dressing combination on the curative effect and inflammatory factors of chronic refractory wounds of diabetic foot.Methods 120 patients with diabetic foot of chronic refractory wounds patients admitted in The Third Hospital of Inner Mongolia Autonomous Region from January 2020 to August 2022 were selected,they were divided into two groups by random number table method:57 cases in control group and 63 cases in observation group.All patients were treated with given anti-infection,blood glucose control and thorough wound debridement after admission,After debridement,only VSD was used to cover the wound in the control group,after debridement,Nano silver dressing was evenly applied on the wound in the observation group,and then VSD was used to cover the wound.When the wound in both groups reached the condition of skin graft,skin grafting was given.The wound healing rate,velocity of wound healing,wound edge reaction and exudation volume,infl ammatory factors and curative eff ect index were compared between the two groups.Results The wound healing rate,velocity of wound healing,infl ammatory factors and curative eff ect indexes of the observation group were signifi cantly higher than those of the control group(P<0.05),while the wound reaction and exudation volume of the observation group were signifi cantly lower than those of the control group(P<0.05).Conclusion VSD combined with Nano silver dressing can eff ectively promote the growth of granulation tissue,shorten the time of wound healing and improve the quality of wound healing,and eff ectively improve the survival rate of diabetic chronic refractory wound skin grafting.
