选择性输卵管造影联合介入再通术对近端输卵管性不孕的诊疗价值分析

摘　　要：目的评价选择性输卵管造影(SSG)联合介入再通术对近端输卵管性不孕诊治的有效性与安全性。方法回顾性分析36例近端输卵管性不孕患者的临床资料,均进行选择性输卵管造影联合介入再通术诊疗。记录选择性输卵管造影技术成功率和手术后12个月内的妊娠成功率。结果36例接受选择性输卵管造影介入再通术患者,平均手术时间为19 min(16~28 min),平均透视时间为51.2 s(38.2~96.5 s)。输卵管全程清晰显影,可见对比影剂均匀地弥散并进入盆腔,表示输卵管通畅。近端输卵管梗阻实施选择性造影技术的成功率为100%,显示输卵管通而不畅双侧13例,单侧23例,共有49条阻塞输卵管,42条被重新开通,成功率为85.7%。经过6~12个月的术后随访,总体妊娠成功率为38.9%(14/36)。部分患者除轻度阴道出血外,没有严重并发症发生(如过敏反应、严重疼痛、迷走反射、子宫输卵管穿孔等),无术后感染发生。结论选择性输卵管造影和介入再通术对于近端输卵管性不孕患者是一种安全有效的方法,技术成功率高,为不孕女性在选择其他受孕方式前,提供一种可选择的治疗方法。Objective To evaluate the effectiveness and security of selective salpingography(SSG)combined with interventional recanalization in the diagnosis and treatment of proximal tubal infertility.Methods The clinical data of 36 patients with proximal tubal infertility were retrospectively analyzed,all of whom were treated with selective salpingography combined with interventional recanalization.The success rate of selective salpingography and pregnancy success rate within 12 months after surgery were recorded.Results The mean procedure time was 19 min(16-28 min)and the mean fluoroscopy time was 51.2 s(38.2-96.5 s)in 36 patients who underwent selective salpingography and interventional recanalization.The fallopian tubes were clearly visualized throughout,and the contrast agent was seen to diffuse uniformly and enter the pelvis,indicating patency of the fallopian tubes.The success rate of selective angiography for proximal tubal obstruction was 100%,showing that the fallopian tube was partially obstructed in 13 bilateral cases and 23 unilateral cases,with a total of 49 obstructed tubes,42 of which were reopened,with a success rate of 85.7%.After 6-12 months of follow-up,the overall pregnancy success rate was 38.9%(14/36).In addition to mild vaginal bleeding,some patients had no serious complications(such as allergic reaction,severe pain,vagal reflex,uterine and fallopian tube perforation,etc.),and no postoperative infection occurred.Conclusion Selective slappingography and interventional recanalization is a safe and effective method with a high success rate,which provides an alternative treatment for infertile women before choosing other conception methods.
