自动弹力线套扎术治疗痔疮的临床效果观察

摘　　要：目的观察自动弹力线套扎术(RPH)治疗痔疮的临床疗效。方法170例痔疮患者为研究对象,根据治疗方法不同分为观察组(120例)及对照组(50例)。观察组采用自动弹力线套扎术治疗,对照组采用吻合器痔上黏膜环切术(PPH)治疗。比较两组患者的疗效,并发症发生情况,术后疼痛评分。结果两组患者的总有效率均为100.00%,比较差异无统计学意义(P>0.05)。观察组患者的术中出血发生率4.17%低于对照组的36.00%,差异具有统计学意义(P<0.05);两组患者的术后出血、肛门狭窄、肛门坠胀发生率比较,差异无统计学意义(P>0.05)。两组均未发生术后水肿。观察组患者的术后视觉模拟评分法(VAS)评分为(2.0±0.6)分,与对照组的(2.0±0.8)分比较,差异无统计学意义(P>0.05)。结论自动弹力线套扎术操作简单,易于学习,并发症少,安全性高,住院时间短,恢复快,患者接受度高。吻合器痔上黏膜环切术操作相对复杂,学习曲线较长,术后遗留钛钉,并发症处理较复杂,患者接受度较低,逐渐被自动弹力线套扎术替代。Objective To observe the clinical efficacy of ruiyun procedure for hemorrhoid(RPH)in the treatment of hemorrhoids.Methods A total of 170 patients with hemorrhoids as study subjects were divided into observation group(120 cases)and control group(50 cases)according to different treatment methods.The observation group was treated with ruiyun procedure for hemorrhoid,and the control group was treated with procedure for prolapse and hemorrhoids(PPH).The efficacy,complications,and postoperative pain score were compared between the two groups.Results The total effective rate of both groups was 100.00%,and the difference between the two groups was not statistically significant(P>0.05).The incidence of intraoperative bleeding in the observation group was 4.17%,which was lower than that of 36.00%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of postoperative bleeding,anal stenosis,and anal distention between the two groups(P>0.05).No postoperative edema occurred in both groups.The postoperative visual analogue scale(VAS)score of the observation group was(2.0±0.6)points,and the difference was not statistically significant when compared with that of(2.0±0.8)points of the control group(P>0.05).Conclusion Ruiyun procedure for hemorrhoid is simple to perform,easy to learn,has few complications,high safety,short hospital stay,quick recovery,and high patient acceptance.Procedure for prolapse and hemorrhoids is relatively complicated to perform,has a long learning curve,postoperative titanium nail left behind,complicated complication management,and low patient acceptance,which has been gradually replaced by ruiyun procedure for hemorrhoid.
