摘 要:目的观察慢性阻塞性肺疾病(慢阻肺)合并社区获得性肺炎采用抗生素联合(阿莫西林钠克拉维酸钾联合左氧氟沙星)治疗的临床效果。方法100例慢阻肺合并社区获得性肺炎患者为研究对象,采用随机数字表法将其分为抗生素单一用药组与抗生素联合用药组,各50例。抗生素单一用药组采用阿莫西林钠克拉维酸钾治疗,抗生素联合用药组采用阿莫西林钠克拉维酸钾联合左氧氟沙星治疗。对比两组治疗前后炎性指标及症状评分,肺部病灶吸收效果,不良反应发生情况。结果治疗后,抗生素联合用药组WBC(6.41±1.03)×10^(9)/L、CRP(15.19±1.17)mg/L、PCT(0.41±0.11)μg/L均低于抗生素单一用药组的(9.31±0.11)×10^(9)/L、(21.12±0.12)mg/L、(0.71±0.12)μg/L,差异有统计学意义(P<0.05)。治疗后,抗生素联合用药组发热、咳喘症状评分均低于抗生素单一用药组,差异有统计学意义(P<0.05)。抗生素联合用药组肺部病灶吸收总有效率96.00%高于抗生素单一用药组的82.00%,差异有统计学意义(P<0.05)。治疗后,两组用药不良反应发生率对比,差异无统计学意义(P>0.05)。结论慢阻肺合并社区获得性肺炎患者接受抗生素联合(阿莫西林钠克拉维酸钾联合左氧氟沙星)方案治疗能够快速降低患者炎性指标,改善临床症状,提升治疗效果。Objective To observe the clinical effect of antibiotic combination therapy(amoxicillin and clavulanate potassium in combination with levofloxacin)for chronic obstructive pulmonary disease complicated with community-acquired pneumonia.Methods A total of 100 patients with COPD and community-acquired pneumonia were divided into single antibiotic treatment group and combined antibiotic treatment group according to the random numerical table,with 50 cases in each group.Single antibiotic group was treated with amoxicillin and clavulanate potassium,and combined antibiotic group was treated with amoxicillin and clavulanate potassium in combination with levofloxacin.The inflammatory indexes and symptom scores before and after treatment,lung lesion absorption effect,and adverse reactions were compared between the two groups.Results After treatment,combined antibiotic treatment group had WBC of(6.41±1.03)×10^(9)/L,CRP of(15.19±1.17)mg/L,PCT of(0.41±0.11)μg/L,CRP of(15.19±1.17)mg/L,and PCT of(0.41±0.11)μg/L,which were lower than those of(9.31±0.11)×10^(9)/L,(21.12±0.12)mg/L,and(0.71±0.12)μg/L in single antibiotic group,and the differences were statistically significant(P<0.05).After treatment,the scores of fever,cough and asthma in the combined antibiotic treatment group were lower than those of the single antibiotic treatment group,and the differences were statistically significant(P<0.05).The total effective rate of lung lesion absorption in combined antibiotic treatment group was 96.00%,which was higher than that of 82.00%in single antibiotic treatment group,and the difference was statistically significant(P<0.05).After treatment,there was no statistically significant difference in the incidence of adverse drug reactions between the two groups(P>0.05).Conclusion Antibiotic combination therapy for patients with chronic obstructive pulmonary disease and community-acquired pneumonia can rapidly reduce the inflammatory indexes of patients,improve the clinical symptoms,and enhance the therapeutic effect.
