摘 要:目的探讨高位SMAS除皱术在面部松垂患者中的应用效果。方法自2021年8月至2022年9月,哈尔滨医科大学附属第四医院(松北院区)整形美容外科和哈尔滨医科大学附属第二医院整形外科对32例面部松垂的患者实施高位SMAS手术治疗,取颞部发际缘、耳面交界处切口,皮下剥离完成后,在颧弓上缘水平切开进入到SMAS瓣下,离断面部支持韧带,掀起SMAS瓣,向上提拉逐级减张缝合固定。结果32例患者,其中1例于术后第2天右侧术区出现血肿,给予局部抽吸后继续加压包扎至痊愈,其余所有患者随访6~12个月,鼻唇沟加深和下颌缘松垂症状均改善明显,无面神经损伤的并发症发生,效果满意。结论高位SMAS除皱可有效改善衰老的鼻唇沟和下颌缘形态,术后效果持久,并发症较少。Objective To explore the application of high SMAS technology in the facial rhytidecotomy.Methods From August 2021 to September 2022,32 patients underwent high SMAS surgery in this study.Incision was made along the temporal hairline and pre-auricular area,after completing subcutaneous dissection,the SMAS was incised transversely at the upper edgethe arch.SMAS falp was elevated and fixed after the facial retaining ligament were amputated.Results There are 32 patients in this group,1 of them developed a hematoma in the right surgical area on the second day after surgery,and continued to compress and bandage until healed after local aspiration.Afer 6~12 months of follow-up,all patients in this group had signigicant improvement on nasolabial fold and mandibular margin sagging.One patient developed a hematoma in the right surgical area the second day later after the operation,recovery occurred after local treatment,the effect was satisfactory.ConclusionHigh SMAS technique can improve effectively the aging appearance of middle face with long-lasting postoperative effect,and no facial nerve complications occurred.
