摘　　要：目的 评价环泊酚复合阿芬太尼用于糖尿病(diabetes mellitus,DM)患者无痛胃镜检查术的优化效应。方法 纳入郑州大学第一附属医院择期接受无痛胃镜检查术的DM患者88例,采用随机数字表法分为环泊酚复合阿芬太尼组(C组)和丙泊酚复合阿芬太尼组(P组),每组44例。C组依次静脉注射阿芬太尼、环泊酚,P组依次静脉注射阿芬太尼、丙泊酚。记录镇静成功率、镇静起效时间、苏醒时间、离院时间及不良事件发生率,以及麻醉医师、内镜医师和患者满意度。结果P组较C组术中心动过缓、低血压、呼吸抑制、注射痛发生率低(22.73%vs 9.09%,56.89%vs 20.45%,38.64%vs 6.82%,59.09%vs 4.55%)(P <0.05),麻醉医生及患者满意度升高[(6.82±0.80)分vs (8.03±0.60)分;(8.82±0.70)分vs (9.44±1.20)分](P <0.05)。两组镇静成功率、镇静起效时间、苏醒时间和离院时间差异均无统计学意义(P> 0.05)。结论 与常规麻醉相比,环泊酚复合阿芬太尼用于DM患者无痛胃镜检查术时,在安全性及有效性方面有一定优化效应。Objective To evaluate the optimization effects of ciprofol combined with alfentanil for painless gastroscopy in patients with diabetes mellitus(DM).Methods A total of 88 DM patients who underwent painless gastroscopy in the first affiliated hospital of Zhengzhou university were enrolled and randomly divided into ciprofol combined with alfentanil group(group C)and propofol combined with alfentanil group(group P)by random number table method,with 44 cases in each group.Group C received intravenous injection of alfentanil and ciprofol,and group P received intravenous injection of alfentanil and propofol.The success rate of sedation,duration of sedation onset,time to recovery,time to discharge,incidence of adverse events,and satisfaction of anesthesiologists,endoscopists,and patients were recorded.Results The incidence of bradycardia,hypotension,respiratory depression and injection pain in group P was lower than that in group C(22.73%vs 9.09%,56.89%vs 20.45%,38.64%vs 6.82%,59.09%vs 4.55%,P<0.05).Compared with group C,the anesthesiologists and patients satisfaction in group P increased[(6.82±0.80)vs(8.03±0.60),(8.82±0.70)vs(9.44±1.20),(P<0.05)].Conclusion Compared with conventional anesthesia,ciprofol combined with alfentanil can optimize the safety and effectiveness of painless gastroscopy in DM patients.
