通腑泄热复元汤治疗脓毒症胃肠功能障碍临床观察

摘　　要：目的:观察通腑泄热复元汤对脓毒症胃肠功能障碍患者肠道功能、炎症反应水平及临床疗效的影响。方法:选取75例脓毒症胃肠功能障碍患者,以随机数字表法分为观察组38例、对照组37例,对照组给予常规西医治疗,观察组在对照组基础上给予通腑泄热复元汤治疗,治疗7 d后比较两组患者治疗前后肠道功能障碍症状评分、肠道黏膜屏障功能指标、炎症反应水平,并对两组患者临床疗效进行评估。结果:治疗后两组肠道功能障碍症状各项评分均较治疗前降低(P<0.05),肠道黏膜屏障功能指标D-乳酸、二胺氧化酶均较治疗前降低(P<0.05),炎症反应指标C反应蛋白、白细胞介素-6、肿瘤坏死因子-α水平均较治疗前降低(P<0.05),且观察组各项评分及各指标水平均低于对照组(P<0.05)。观察组临床总有效率94.74%,对照组78.38%,两组临床疗效比较,差异有统计学意义(P<0.05)。结论:通腑泄热复元汤治疗脓毒症胃肠功能障碍临床疗效显著,可有效改善患者肠道功能及肠道黏膜屏障功能,减轻患者炎症反应。Objective:To observe the effect of Tongfu Xiere Fuyuan Decoction(通腑泄热复元汤)on the intestinal function,inflammatory response level,and clinical efficacy in treating sepsis gastrointestinal dysfunction.Methods:Total 75 patients with sepsis gastrointestinal dysfunction were selected and divided into the observation group of 38 cases and the control group of 37 cases according to the random number table method.The control group was treated with conventional western medicine,while the observation group was treated with Tongfu Xiere Fuyuan Decoction on the basis of the treatment of the control group.After 7 days of treatment,the scores of intestinal dysfunction symptoms,intestinal mucosal barrier function indexes,and inflammatory response levels of the two groups were compared before and after treatment,and the clinical efficacy of the two groups was evaluated.Results:After treatment,the scores of intestinal dysfunction symptoms of both groups decreased compared to those before treatment(P<0.05).The intestinal mucosal barrier function indexes D-lactate and diamine oxidase were lower than before treatment,while inflammatory response indicators,such as C-reactive protein,interleukin-6 and tumor necrosis factor-α,were also decreased compared to before treatment(P<0.05).The scores and levels of indexes in the observation group were lower than those in the control group(P<0.05).The total clinical effective rate of the observation group was 94.74%,while that of the control group was 78.38%.The difference in clinical efficacy between the two groups was statistically significant(P<0.05).Conclusions:Tongfu Xiere Fuyuan Decoction has a significant clinical efficacy in treating sepsis gastrointestinal dysfunction,which can effectively improve the intestinal function and intestinal mucosal barrier function of patients,and reduce inflammatory response.
