摘 要:目的:研究早期糖尿病肾病(EDN)中尿微量白蛋白/肌酐比值(尿ACR,UACR)与C1q-肿瘤坏死因子相关蛋白13(CTRP13)和平均红细胞容积(MCV)的相关性。方法:收集2018年6月—2019年8月住院的2型糖尿病患者46例,根据UACR分为2型糖尿病组(T2DM组)20例(UACR<30mg/g)和早期糖尿病肾病组(EDN组)26例(30mg/g≤UACR≤300mg/g),并同期在体检中心收集正常对照组(CG组)30例,检测各组的血细胞分析如红细胞计数(RBC)、白细胞(WBC)、血小板(PLT)等及血清CTRP13和生化指标如血脂和肾功能,进行三组临床资料的比较,并以UACR为应变量,其他多个指标为自变量,进行多元线性回归分析。结果:三组方差分析结果显示:MCV、甘油三脂(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、肌酐(Cr)、空腹血糖(FBG)、UACR、CTRP13等有统计学差异(P<0.05),其中MCV在CG组中最大(92.85±4.08)fl,T2DM组已明显下降(89.62±3.32)fl,EDN组进一步下降(87.68±3.68)fl(F=13.356,P<0.01);CTRP13在T2DM组(38.27±15.79)ng/ml,低于CG组(42.45±13.16)ng/ml,但到EDN组(53.14±18.29)ng/ml极度增高(P<0.05)。Pearson相关分析显示:UACR与MCV、平均红细胞血红蛋白量(MCH)、CTRP13、TG、TC、Cr、FBG相关。UACR的多元线性回归分析:MCV、Cr、CTRP13是UACR的危险因素。结论:随着UACR的增大,MCV呈现下降趋势,而UACR与Cr、CTRP13呈正相关,MCV、CTRP13可能成为早期糖尿病肾病的预测指标。Objective:To study the correlation between urinary microalbumin/creatinine ratio(UACR)and C1q tumor necrosis factor associated protein 13(CTRP13)and mean corpuscular volume(MCV)in early diabetic nephropathy.Methods:46 patients with type 2 diabetes who were hospitalized from June 2018 to August 2019 were divided into two groups according to UACR:20 patients with type 2 diabetes mellitus(T2DM group,UACR<30mg/g)and 26 patients with early diabetic nephropathy(EDN group,30mg/g≤UACR≤300mg/g).At the same time,30 normal control groups(CG groups)were collected in the physical examination center.Blood cell analysis,such as RBC,WBC,PLT,serum CTRP13 and biochemistry were detected.Indexes such as blood lipid and renal function were compared in the three groups.UACR was used as the corresponding variable and other indexes as the independent variable.Results:The results of variance analysis showed that there were significant differences in MCV,TG,TC,HDL-C,Cr,FBG,ACR and CTRP13 in the three groups(P<0.05).Among them,MCV was the largest in the CG group(92.85±4.08)fl,and the T2DM group had significantly decreased(89.62±3.32)fl,and the EDN group of diabetic nephropathy decreased further(87.68±3.68)fl(F=13.356,P<0.01);CTRP13 in T2DM group(38.27±15.79)ng/ml,lower than that in the CG group(42.45±13.16)ng/ml,but it was extremely higher in the EDN group(53.14±53.14)ng/ml.Pearson correlation analysis showed that UACR was related to MCV,MCH,CTRP13,TG,TC,Cr and FBG.Multiple regression analysis of UACR:MCV,Cr,CTRP13 are closely related to UACR.Conclusion:With the increase of UACR,MCV shows a downward trend,while UACR is positively correlated with Cr and CTRP13.MCV and CTRP13 may be the predictors of the early diabetic nephropathy.
