摘　　要：目的:观察桃红四物汤加减联合助卵泡汤分阶段口服对体外受精-胚胎移植(IVF-ET)中卵巢低反应(POR)患者卵巢反应性及卵子质量的影响。方法:将110例IVF-ET中卵巢低反应患者根据治疗方式不同分为58例对照组(常规西药治疗)和52例观察组(对照组基础上+桃红四物汤加减联合自拟助卵泡汤)。比较两组间中医症状积分,HCG注射日雌二醇(E_(2))、孕酮(P)和抗苗勒氏管激素(AMH)水平,记录两组生化妊娠率和临床妊娠率及不良反应发生情况。结果:治疗后,两组中医症状积分均降低(P<0.05),且观察组中医症状积分显著低于对照组(P<0.05);注射HCG当日,观察组E_(2)和AMH显著高于对照组(P<0.05),P水平显著低于对照组(P<0.05);观察组临床妊娠率显著高于对照组(P<0.05),两组间生化妊娠率比较差异无统计学意义(P>0.05);两组均未发生明显不良反应。结论:桃红四物汤与助卵泡汤联合应用于IVF-ET中POR患者,能有效改善患者临床症状,提高卵巢反应性及卵子质量,改善妊娠结局。Objective:To observe the effects of phased oral administration of modified Taohong Siwu decoction combined with Zhuluanpao decoction on ovarian response and oocyte quality in patients with low ovarian response during in vitro fertilization and embryo transfer(IVF-ET).Methods:The clinical data of 110 patients with low ovarian response during IVF-ET were retrospectively analyzed,and the patients were divided into 58 cases in control group(conventional western medicine treatment)and 52 cases in observation group(modified Taohong Siwu decoction combined with Zhuluanpao decoction on the basis of control group)according to the different treatment methods.TCM symptoms scores,levels of estradiol(E_(2)),progesterone(P)and anti-Mullerian hormone(AMH)on the day of HCG injection.Biochemical pregnancy rate and clinical pregnancy rate as well as occurrence of adverse reactions were recorded in the two groups.Results:After treatment,the TCM symptoms scores in both groups were decreased(P<0.05),and the scores in observation group were significantly lower than those in control group(P<0.05).On the day of HCG injection,the levels of E_(2) and AMH in observation group were significantly higher than those in control group(P<0.05),while the P level was significantly lower than that in control group(P<0.05).The clinical pregnancy rate in observation group was significantly higher than that in control group(P<0.05),but there was no statistical significance in biochemical pregnancy rate between the two groups(P>0.05).No obvious adverse reactions occurred in the two groups.Conclusion:Taohong Siwu decoction combined with Zhuluanpao decoction can effectively improve the clinical symptoms,enhance the ovarian response and oocyte quality,and promote the pregnancy outcomes in patients with POR during IVF-ET.
