精子DNA碎片指数和高可染性指数对男性不育患者生育能力的诊断效能

摘　　要：目的:探讨精子DNA碎片指数(DFI)和高可染性指数(HDS)对男性不育症的诊断效能。方法:选取不育症男性患者87例为观察组,另选取同期体检健康的同年龄段男性80例为对照组。比较两组各项精液参数的差异。采用pearson法分析精子DFI、HDS与其他常规参数的相关性。采用受试者工作特征(ROC)曲线分析DFI、HDS及其他常规参数对男性不育症的诊断效能。结果:观察组精子液化时间、DFI、HDS高于对照组,其他常规参数低于对照组(均P<0.05)。精子DFI和HDS与精子活动率、a+b级活动力、精子运动直线速度、精子运动曲线速度、平均路径速度、正常精子形态率呈负相关(均P<0.05)。精子总数、精子浓度、精子活动率、a+b级活动力、精子运动直线速度、精子运动曲线速度、平均路径速度、正常精子形态率及联合检测对男性不育症均有一定的诊断效能。相对于精子常规参数,DFI、HDS单独及联合对男性不育症的诊断效能更高,且联合高于单项。结论:精子DFI和HDS对男性不育症具有较高的诊断效能。Objective:To investigate the diagnostic efficacy of sperm DNA-Fragmentation-Index(DFI)and High DNA Stainability(HDS)in male infertility.Methods:A total of 87 male patients with infertility were selected as the observation group,and 80 male patients of the same age who were healthy during the same period were selected as the control group.The differences of semen parameters between the two groups were compared.The correlation between DFI,HDS and other conventional parameters was analyzed by pearson method.ROC curve was used to analyze the diagnostic efficacy of DFI,HDS and other conventional parameters in male infertility.Results:Sperm liquefaction time,DFI and HDS in observation group were higher than those in control group,and other conventional parameters were lower than those in control group(all P<0.05).Sperm DFI and HDS were negatively correlated with sperm motility rate,a+b level motility,sperm linear velocity,sperm curve velocity,average path velocity and normal sperm morphology rate(all P<0.05).The total sperm count,sperm concentration,sperm activity rate,a+b level activity,sperm linear velocity,sperm movement curve velocity(AUC=644),average path velocity,normal sperm morphology rate and combination of them had certain diagnostic efficacy for male infertility.Compared with the conventional parameters of sperm,DFI and HDS alone and combined were more effective in the diagnosis of male infertility.Conclusion:Sperm DFI and HDS have high diagnostic efficacy for male infertility.
