摘 要:目的:探讨特应性皮炎(AD)患儿血清长链非编码RNA母系表达基因3(lncRNA MEG3)和微小RNA-23b-3p(miR-23b-3p)水平检测及意义。方法:选取AD患儿165例(AD组)和体检健康儿童160例(对照组)为研究对象。采用荧光定量PCR法检测两组血清lncRNA MEG3和miR-23b-3p水平。采用AD积分指数(SCORAD)对AD患儿疾病严重程度进行评估,并将AD组患儿分为AD轻度组(65例)、AD中度组(68例)和AD重度组(32例)。比较对照组和AD组以及不同严重程度AD患儿血清lncRNA MEG3、miR-23b-3p水平。采用Pearson法分析AD组血清lncRNA MEG3、miR-23b-3p水平和SCORAD评分三者间的相关性。采用受试者工作特征(ROC)曲线分析血清lncRNA MEG3、miR-23b-3p水平检测对中、重度AD的诊断价值。结果:与对照组比较,AD组血清lncRNA MEG3和miR-23b-3p水平显著降低(均P<0.05)。与AD轻度组比较,AD中度组和AD重度组血清lncRNA MEG3、miR-23b-3p水平依次降低,SCORAD评分依次升高(均P<0.05)。AD组血清lncRNA MEG3、miR-23b-3p水平呈正相关(r=0.404,P<0.05)。血清lncRNA MEG3、miR-23b-3p水平与SCORAD评分均呈负相关(r=-0.383、-0.366,均P<0.05)。血清lncRNA MEG3、miR-23b-3p单项及联合检测诊断中度AD的曲线下面积(AUC)分别为0.818、0.753、0.883。与血清lncRNA MEG3、miR-23b-3p单独检测比较,两者联合检测诊断中度AD的AUC更高(Z=2.177、3.595,均P<0.05)。血清lncRNA MEG3、miR-23b-3p单项及联合检测诊断重度AD的AUC分别为0.794、0.778、0.895。与血清lncRNA MEG3、miR-23b-3p单独检测比较,两者联合检测诊断重度AD的AUC更高(Z=2.104、2.293,均P<0.05)。结论:AD患儿血清lncRNA MEG3和miR-23b-3p水平呈低表达,与病情严重程度有关,有望成为评估儿童AD病情程度的生物标志物。Objective:To explore the detection and significance of serum long non-coding RNA(lncRNA)maternal expression gene 3(MEG3)and microRNA-23b-3p(miR-23b-3p)in children with atopic dermatitis(AD).Methods:A total of 165 children with AD(AD group)and 160 healthy children who underwent physical examination(control group)were enrolled as study subjects.Serum lncRNA MEG3 and miR-23b-3p levels were detected by fluorescence quantitative PCR.The severity of AD was evaluated by using AD Integral Index(SCORAD)score,and AD children were divided into mild AD group(65 cases),moderate AD group(68 cases)and severe AD group(32 cases).The serum levels of lncRNA MEG3 and miR-23b-3p were compared between the control group and AD group,and among children with different severity of AD.Pearson method was used to analyze the correlation between serum levels of lncRNA MEG3 and miR-23b-3p and SCORAD score in AD group.The diagnostic value of serum lncRNA MEG3 and miR-23b-3p levels in moderate and severe AD was analyzed by subject operating characteristic curve.Results:Compared with control group,serum lncRNA MEG3 and miR-23b-3p levels in AD group were significantly decreased(all P<0.05).Serum lncRNA MEG3 and miR-23b-3p levels in mild AD group,moderate AD group and severe AD group were decreased successively,while SCORAD score was increased successively(all P<0.05).Serum lncRNA MEG3 and miR-23b-3p levels in AD group were positively correlated(r=0.404,P<0.05),and serum lncRNA MEG3 and miR-23b-3p levels were negatively correlated with SCORAD score(r=-0.383,-0.366,all P<0.05).The AUC of serum lncRNA MEG3 and miR-23b-3p single and combined detection in the diagnosis of moderate AD were 0.818,0.753 and 0.883,respectively.Compared with the single detection of serum lncRNA MEG3 and miR-23b-3p,the AUC of the combined detection of serum lncRNA MEG3 and miR-23b-3p in the diagnosis of moderate AD was higher(Z=2.177,3.595,all P<0.05).The AUC of serum lncRNA MEG3 and miR-23b-3p single and combined detection in the diagnosis of severe AD were 0.794,0.778 and
