基于六经、八纲辨证治疗结直肠癌体会

摘　　要：六经辨证是将疾病发展传变过程中发病的部位、病性的寒热虚实、脉象和证候的变化进行综合判定,将疾病分为太阳、阳明、少阳、少阴、太阴、厥阴六经的辨证方法。八纲辨证是将四诊取得的信息综合分析,以探求疾病的性质、病变部位、病势轻重、机体反应的强弱、正邪双方力量的对比等情况,将疾病归纳为阴、阳、表、里、寒、热、虚、实八类证候,八纲辨证的思想源于六经辨证。结直肠癌的治疗原则为扶正祛邪、攻补兼施。临证时应立足于六经辨证和八纲辨证,若患者病情日久,气血亏虚,加之受寒受风,可予桂枝汤;若患者中焦气滞不行,气逆于上,可予半夏泻心汤、甘草泻心汤;若患者潮热盗汗,干咳少痰,可选竹叶石膏汤、麦门冬汤、生脉散;若患者腹胀不适,口苦咽干,可予柴胡类方;若患者中焦寒湿,腹痛呕吐,可予理中汤。The Liujing differentiation is a comprehensive assessment of the location of disease development,the nature of the disease(Cold,Heat,deficiency,excess),pulse condition and changes in symptoms.It classifies diseases into Liujing(six meridians):Taiyang,Yangming,Shaoyang,Shaoyin,Taiyin and Jueyin.The Bagang(eight-principles)differentiation analyzes information obtained from the four diagnostic methods to determine the nature of the disease,the location of the pathology,the severity of the condition,the strength of the body′s response and the balance between positive and negative forces.It categorizes diseases into eight types:Yin,Yang,Exterior,Interior,Cold,Heat,Deficiency,and Excess.The concept of eight-principle differentiation originates from six-meridian differentiation.The treatment principles for colorectal cancer are tonifying the body′s vitality while eliminating pathogenic factors.When diagnosing,it is important to consider both six-meridian differentiation and eight-principle differentiation.For patients with long-standing illness such as being deficient Qi and blood,being aggravated by Cold and Wind,Guizhi Decoction can be used.For patients with Qi stagnation in the middle burner and Qi rebelliousness upward,Banxia Xiexin Decoction or Gancao Xiexin Decoction may be used.For patients with Hot flashes,night sweats or dry cough with little Phlegm,Zhuye Shigao Decoction,Maimendong Decoction and Shengnai San Powder can be used.For patients with abdominal distension kind of discomfort,bitter taste in the mouth and dry throat,Chaihu Decoction kinds of formula may be used.For patients with Cold and Dampness in the middle burner,abdominal pain or vomiting,Lizhong Decoction may be used.
