中医药促进股骨头坏死血管修复与生成的研究进展

摘　　要：中医药可通过调控磷脂酰肌醇3-激酶(phosphoinositide3-kinase,PI3K)/苏氨酸激酶(threonine kinase,AKT)通路、Notch通路、缺氧诱导因子-1α(hypoxia inducible factor-1α,HIF-1α)通路、微小RNA(micro RNA,miRNA)、转化生长因子-β(transforming growth factor-β,TGF-β)、骨形成发生蛋白-2(bone morphogenetic protein 2,BMP-2)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、炎症因子等多条途径促进股骨头坏死血管修复与生成,改善局部血供,延缓病情进展,但目前中医药治疗股骨头坏死面临着周期长、见效慢、易发生肝肾功能损害等问题,后期可联合髓芯减压、打压植骨等方法共同治疗。未来也可将中医学与循证医学联合,在辨证论治的同时结合现代科学理论进行分析,为中医药治疗股骨头坏死提供客观、可重复的理论依据。Traditional Chinese medicine can regulate phosphoinositide3-kinase(PI3K)/threonine kinase(AKT)pathway,Notch pathway,hypoxia inducible factor-1α(HIF-1α)pathway,micro RNA,(MiRNA),transforming growth factor-β(TGF-β),bone morphogenetic protein-2(BMP-2),vascular endothelial growth factor(VEGF),VEGF,inflammatory factors and other ways to promote the repair and generation of blood vessels of femoral head necrosis,improve local blood supply,and delay the progress of the disease.However,the treatment of femoral head necrosis with traditional Chinese medicine alone faces problems such as long cycle,slow effect and easy occurrence of Liver and Kidney function damage,and it can be combined with core decompression and bone grafting in the later stage.In the future,we can also combine traditional Chinese medicine with evidence-based medicine and analyze it with modern scientific theory while treating it according to syndrome differentiation so as to provide an objective and repeatable theoretical basis for the treatment of femoral head necrosis with traditional Chinese medicine.
