摘　　要：为了有效地减轻老年人的主要疾病负担,利用互联网+技术的便捷性和普及性建设一个管理流程、健康数据全闭环的院级老年常见病服务平台意义重大。本文提出了一个可同时适用于各级各类医疗机构和社区卫生院实现院级全闭环的老年常见病服务平台的建设方案,对老年病患者的闭环管理流程和健康数据采集闭环管理的实现提出了具体的建设措施。In order to effectively reduce the burden of major diseases of the elderly,it is of great significance to use the convenience and popularity of Internet plus technology to build a hospital level service platform for common diseases of the elderly with a full closed-loop management process and health data.This article proposes a construction plan for a hospital level full closed-loop elderly common disease service platform that can be applied to all levels and types of medical institutions and community health centers.Specific construction measures are proposed for the closed-loop management process of elderly patients and the implementation of health data collection closed-loop management.
