男性尖锐湿疣患者HPV基因型和临床特征及其复发的危险因素

摘　　要：目的分析男性尖锐湿疣(CA)感染人乳头瘤病毒(HPV)基因型和临床特征及其复发的危险因素。方法采集2020年1月-2021年1月武汉市第一医院119例男性CA患者疣体组织,提取DNA,使用人乳头瘤病毒(HPV)核酸分型检测试剂盒对样本进行基因型检测,分析并比较患者临床特征和HPV复发、感染情况,查阅患者病历资料,归纳CA复发的危险因素。结果119例男性CA患者共100例检出HPV基因型别,阳性检出率为84.03%,其中高危型HPV 55种(55.00%),低危型HPV 66种(66.00%),多重感染HPV 41种(41.00%)。接受物理治疗后复发率为36.97%(44/119),复发组检出高危型35种(79.55%),低危型12种(27.27%),多重感染25种(56.82%),未复发组检出高危型20种(26.67%),低危型54种(72.00%),多重感染16种(21.33%),两组高危型HPV、低危型HPV、多重感染HPV差异均有统计学意义(P<0.05);多因素Logistic回归结果显示,未使用安全套、肛周疣、高危型HPV、HPV多重感染是CA复发的危险因素(P<0.05)。结论男性CA感染HPV基因型以HPV6、HPV11、HPV16、HPV18为主,临床多为经典型CA,40岁以下较多发,未使用安全套、肛周疣、高危型HPV、HPV多重感染是CA复发危险因素。OBJECTIVE To analyze the genotype and clinical characteristics of human papillomavirus(HPV)in male patients with condyloma acuminatum(CA)and its risk factors for recurrence.METHODS Verruca tissues were collected from 119 male patients with CA who were admitted to the Wuhan First Hospital from Jan.2020 to Jan.2021,DNA was extracted,the genotypes of samples were tested by HPV DNA test kits.The patients'clinical characteristics,HPV recurrence and infection were analyzed and compared.The medical records of patients were reviewed,and risk factors for CA recurrence were summarized.RESULTS Among the 119 patients with CA,a total of 100 patients were detected for HPV genotypes,with a positive detection rate of 84.03%,including 55(55.00%)kinds of high-risk HPV,66(66.00%)kinds of low-risk HPV and 41(41.00%)kinds of multiple HPV infections.The recurrence rate after physical therapy was 36.97%(44/119),35 kinds of high-risk types(79.55%),12 kinds of low-risk types(27.27%)and 25 kinds of multiple infections(56.82%)were detected in the recurrent group,while 20 kinds of high-risk types(26.67%),54 kinds of low-risk types(72.00%)and 16 kinds of multiple infections(21.33%)were detected in the non-recurrent group,and there were statistically significant differences between the two groups in terms of high-risk HPV infection,low-risk HPV infection and multiple HPV infection(P<0.05).Multivariate logistic regression analysis showed that no use of condoms,perianal verrucas,high-risk HPV and multiple HPV infection were risk factors for recurrence of CA(P<0.05).CONCLUSION HPV6,HPV11,HPV16 and HPV18 were the main HPV genotypes in males with CA infection.Clinically,most of the patients had classic CA,with more frequent occurrence under the age of 40.No use of condoms,perianal verrucas,high-risk HPV and multiple HPV infection were risk factors for recurrence of CA.
