摘  要:目的探讨自然杀伤(NK)细胞活化性受体自然杀伤细胞2族成员D(NKG2D)基因多态性对合并生殖道感染孕产妇妊娠结局的影响。方法选择北京朝阳医院妇产科2020年1月-2021年1月明确生殖道感染的孕产妇108例为研究对象,根据妊娠结局将孕产妇分为母体不良结局组和无不良结局组,根据出生后新生儿情况将其分为新生儿不良结局组和新生儿无不良结局组。收集所有孕产妇年龄、孕次、产次、产前体质量指数(BMI)和妊娠期糖尿病情况。提取外周血红细胞中基因组DNA,以聚合酶链反应-限制性片段长度多态性方法检测NKG2D基因rs1049174(G/C)位点基因分型。结果不同母体、新生儿分组的母体年龄、孕次、产次、产前BMI、妊娠期合并症及生殖道感染类型情况比较差异均无统计学意义;母体不良结局组、母体无不良结局组、新生儿不良结局组和新生儿无不良结局组NKG2D基因rs1049174(G/C)位点基因型分布实际频数和理论频数比较差异无统计学意义,均符合哈迪-温伯格平衡;母体不良结局组NKG2D基因rs1049174(G/C)位点携带CC基因型和C等位基因频率高于母体无不良结局组;新生儿不良结局组NKG2D基因rs1049174(G/C)位点携带CC基因型和C等位基因频率高于新生儿无不良结局组(P<0.05)。结论NKG2D基因rs1049174位点基因多态性显著影响合并生殖道感染孕产妇母亲和新生儿结局。OBJECTIVE To investigate the effects of NK cell activating receptor natural killer cell family 2 member D(NKG2D)gene polymorphisms on pregnancy outcomes in pregnant and lying-in women with genital tract infection.METHODS Totally 108 cases of pregnant and lying-in women with confirmed genital tract infection from Jan.2020 to Jan.2021 in department of obstetrics and gynecology of Beijing Chaoyang Hospital were selected as the research subjects.The pregnant women were divided into maternal adverse outcome group and non-adverse outcome group according to the pregnancy outcomes,and were classified into neonatal adverse outcome group and neonatal non-adverse outcome group according to neonatal condition after birth.Maternal age,gravidity,parity,prenatal body mass index(BMI)and gestational diabetes were collected from all pregnant and lying-in women.Genomic DNA erythrocytes were extracted from peripheral blood,and genotyping at the rs1049174(G/C)locus of NKG2D gene was detected by polymerase chain reaction-restriction fragment length polymorphism method.RESULTS There were no statistical differences in maternal age,gravidity,parity,prenatal BMI,complications during pregnancy and genital tract infection type among different maternal and neonatal groups.There were no statistically significant differences in the actual and theoretical frequencies of genotype distribution at rs1049174(G/C)locus of NKG2D gene between maternal adverse outcome group and maternal non-adverse outcome group,and between neonatal adverse outcome group and neonatal non-adverse outcome group,which were in line with Hardy-Weinberg equilibrium.The frequencies of CC genotype and C allele at rs1049174(G/C)locus of NKG2D gene in maternal adverse outcome group were higher than those in maternal non-adverse outcome group,and the frequencies of CC genotype and C allele at rs1049174(G/C)locus of NKG2D gene in neonatal adverse outcome group were higher than those in neonatal non-adverse outcome group(P<0.05).CONCLUSION Gene polymorphism at the rs1049174 locus o
