摘 要:眼后段疾病是导致视力损害及失明的主要原因,其药物治疗主要基于侵入性方式,存在并发症多、不良反应严重和患者依从性差的问题。非侵入性眼后段药物递送则需克服眼部的吸收屏障和限制、增加药物在眼后段组织的渗透性,从而达到提高眼后段组织的药物生物利用度的目的。该研究综述了非侵入性眼后段递药的眼生理屏障和药物递送策略(包括渗透促进剂、纳米技术和离子导入法等)的研究进展,并对临床转化面临的挑战和正在进行临床研究的品种进行了总结,最后展望了非侵入性眼后段递药的前景。Posterior segment diseases are the leading causes of visual impairment and blindness,and their treatments are mainly based on invasive approaches with many complications,serious adverse reactions and poor patient compliance.Non-invasive ocular drug delivery systems should be developed to overcome the ocular absorption barriers,increase drug permeability into posterior segment,so as to improve the drug bioavailability in posterior segment.This paper overviews the ocular barriers and novel non-invasive strategies(including the use of penetration enhancers,nanotechnology and iontophoresis)of drug delivery to the posterior eye segment.Moreover,the challenges of translational medicine and candidate drugs under clinical trial are summarized.Finally,the prospects of non-invasive posterior segment delivery are put forward.
