摘  要:目的探究自动心肌运动定量(automated myocardial motion quantification,aCMQ)技术评价亚临床甲状腺功能减退(subclinical hypothyroidism,SCH)患者左心室功能的可行性,为SCH患者早期心功能的检查提供一种新的方法。方法将56例SCH患者纳入试验组,同期收集24例健康体检者为对照组,两组均使用常规超声心动图及aCMQ技术测量得到相关参数,对所有数据进行统计学分析。结果两组参数对比分析,试验组与对照组的常规超声心动图参数差异没有统计学意义(P>0.05),而aCMQ技术获得的参数LVGLS、LVAP2LS、LVAP3LS、LVAP4LS相比于对照组结果有统计学意义(P<0.01)。TSH与左心室纵向应变参数有显著的相关性与之成正相关,r=0.522、0.464、0.482、0.448(P<0.01),aCMQ技术具有良好一致性及可重复性,纵向应变参数均能预测SCH患者左心室早期心功能受损,但LVGLS相对于其他指标的预测价值最高。结论aCMQ技术可以更准确的探查到SCH患者左室心肌的微小变化,为临床早期评价患者左心室功能的改变提供了一种新的方法。Objective To investigate the feasibility of the automated myocardial motion quantification(aCMQ)technique in evaluating left ventricular function in patients with subclinical hypothyroidism(SCH)and to provide a new method for the early examination of cardiac function in SCH patients.Methods 56 SCH patients were included in the experimental group,and 24 healthy physical examination subjects were collected as the control group during the same period.Relevant parameters were measured by conventional echocardiography and aCMQ technology in both groups,and all data were statistically analyzed.Results Compared with the control group,there was no significant difference in conventional echocardiographic parameters between the test group and the control group(P>0.05),while parameters LVGLS,LVAP2LS,LVAP3LS and LVAP4LS obtained by aCMQ technology were statistically significant compared with the control group(P<0.01).TSH was significantly correlated with left ventricular longitudinal strain parameters and positively correlated with them,r=0.522,0.464,0.482,0.448(P<0.01).The aCMQ technology has good consistency and repeatability,and longitudinal strain parameters can predict early left ventricular function impairment in SCH patients.But LVGLS have the highest predictive value relative to other indicators.Conclusion The aCMQ technique can more accurately detect small changes in the left ventricular myocardium of SCH patients,providing a new method for early clinical evaluation of changes in patient left ventricular function.
