Split-face模式观察Nd:YAG 1064nm皮秒激光联合中药巴布贴治疗黄褐斑的临床疗效

摘　　要：目的探讨Nd:YAG1064nm皮秒激光联合中药巴布贴治疗黄褐斑的有效性和安全性。方法采用半边脸自身对照模式(Split-face模式),收集20例符合纳入标准的黄褐斑患者,左右侧面部均进行Nd:YAG1064nm皮秒激光治疗3次。随机分配一侧面部为观察侧,在激光治疗的基础上,每日1次外敷中药巴布贴1小时;另一侧面部为对照侧,单纯行激光治疗。所有患者建议在整个研究期间严格使用防晒霜(SPF≥30,PA++/+++)及保湿霜。3次治疗结束后评价疗效、满意度及不良反应。结果观察侧与对照侧3次治疗后均有效,观察侧疗效明显优于对照侧;观察侧患者满意度为90%,对照侧患者满意度为70%;治疗前后患者SAS、SDS评分均低于治疗前:整个观察期间治疗部位未出现炎症后色素沉着、色素减退、瘢痕、水疱等严重不良反应。结论Nd:YAG1064nm皮秒激光联合中药巴布贴外敷治疗面部黄褐斑疗效确切,不良反应小,值得临床推广。Objective To investigate the efficacy and safety of Nd:YAG 1064nm picosecond laser combined with traditional Chinese medicine cataplasm patch in the treatment of Melasma.Methods The half face self control mode(Split face mode)was used to collect 20 patients with Melasma who met the inclusion criteria.Nd:YAG 1064nm picosecond laser treatment was performed on both sides for three times.Randomly assign one side of the face as the observation side,and on the basis of laser treatment,apply traditional Chinese medicine cataplasm patch once a day for 1 hour;The other side of the face is the control side,and laser therapy alone is performed.All patients are advised to strictly use sunscreen(SPF≥30,PA++/++)and moisturizer throughout the entire study period.Evaluate the effifficacy,satisfaction,and adverse reactions after three treatments.Result Both the observation side and the control side were effffective after 3 treatments,and the observation side had signifificantly better effifficacy than the control side;The satisfaction rate of patients on the observation side is 90%,while the satisfaction rate of patients on the control side is 70%;Before and after treatment,the SAS and SDS scores of the patients were lower than before treatment:during the entire observation period,there were no serious adverse reactions such as inflflammation,pigmentation,scars,blisters,etc.in the treated area.Conclusion The combination of ultra picosecond laser and traditional Chinese medicine cataplasm patch is effffective in the treatment of facial Melasma,with less adverse reactions,and is worthy of clinical promotion.
