阑尾残端荷包包埋与单纯结扎在腹腔镜阑尾切除术中的应用效果比较

摘　　要：目的:对比分析阑尾残端荷包包埋与单纯结扎在腹腔镜阑尾切除术中的应用价值。方法:抽取2014年1月至2021年12月在郑州市第三人民医院行腹腔镜阑尾切除术的150例患者作为研究对象,按随机数字表法分为观察组与对照组,每组75例。对照组采用单纯结扎,观察组采用阑尾残端荷包包埋。比较两组手术情况(手术时间、术中出血量)、术后恢复情况(术后肛门通气时间、压痛时间、反跳痛时间)、并发症发生率及住院情况(术后住院时间、住院费用)。结果:两组术中出血量比较差异未见统计学意义(P>0.05)。观察组手术时间[(51.19±3.45)min]长于对照组[(43.51±3.06)min],P<0.05。观察组术后肛门通气时间、压痛时间、反跳痛时间均短于对照组(P均<0.05)。观察组并发症发生率(4.00%,3/75)低于对照组(14.67%,11/75),P<0.05。观察组术后住院时间及住院费用均少于对照组(P均<0.05)。结论:与单纯结扎相比,阑尾残端荷包包埋应用于腹腔镜阑尾切除术中,虽会在一定程度上延长手术时间,但会减轻术后疼痛,降低并发症发生率,缩短住院时间,减少医疗费用。Objective To compare and analyze the application value of purse-string suture embedding and simple ligation of appendix stump in laparoscopic appendectomy.Methods One hundred and fifty patients who underwent laparoscopic appendectomy in Zhengzhou Third People's Hospital from January 2014 to December 2021 were selected as the research objects,and they were divided into observation group and control group according to the random number table method,with 75 cases in each group.The control group was treated by simple ligation,while the observation group was treated by purse-string suture embedding of appendix stump.The operation conditions(operation time,intraoperative blood loss),postoperative recovery conditions(postoperative anal ventilation time,tenderness time,rebound pain time),incidence of complications and hospitalization conditions(postoperative hospital stay,hospitalization cost)of the two groups were compared.Results There was no significant difference in intraoperative blood loss between the two groups(P>0.05).The operation time in the observation group was(51.19±3.45)min,longer than the(43.51±3.06)min in the control group(P<0.05).The postoperative anal ventilation time,tenderness time and rebound pain time in the observation group were shorter than those in the control group(all P<0.05).The incidence of complications in the observation group was 4.00%(3/75),lower than the 14.67%(11/75)in the control group(P<0.05).The duration of postoperative hospital stay in the observation group was shorter than that in the control group,and the hospitalization cost was less than that in the control group(P<0.05).Conclusions Compared with simple ligation,the application of purse-string suture embedding of appendix stump in laparoscopic appendectomy can extend the operation time to a certain extent,but it can reduce postoperative pain,reduce the incidence of complications,shorten the length of hospital stay and reduce medical costs.
