摘 要:目的:探讨复方α-酮酸对血液透析患者残余肾功能(RRF)及钙磷代谢的影响。方法:抽取2019年8月至2020年8月于新乡医学院第一附属医院进行血液透析治疗的患者86例,采用随机数字表法将其分为联合组(43例)和对照组(43例)。对照组行简易血液透析治疗,联合组在简易血液透析治疗基础上联合复方α-酮酸治疗。比较两组治疗前后残余肾功能、钙磷代谢水平、炎症因子水平及总不良反应发生率。结果:治疗后,两组RRF低于治疗前(P<0.01),联合组RRF高于对照组(P<0.01)。治疗后,联合组血钙、血磷、钙磷乘积、甲状旁腺激素水平低于治疗前(P<0.01),联合组血钙、血磷、钙磷乘积、甲状旁腺激素含量低于对照组(P<0.01)。治疗后,联合组白细胞介素-6、肿瘤坏死因子-α、超敏C反应蛋白水平低于治疗前(P<0.01),联合组细胞介素6、肿瘤坏死因子-α、超敏C反应蛋白水平低于对照组(P<0.01)。治疗后,联合组总不良反应发生率(6.98%,3/43)与对照组(9.30%,4/43)比较差异未见统计学意义(χ^(2)=0.16,P>0.05)。结论:复方α-酮酸能够显著改善血液透析患者残余肾功能、钙磷代谢,减轻炎症反应,且无其他严重不良反应发生。Objective To investigate the effect of compoundα-ketoacids on the residual renal function(RRF)and calcium-phosphorus metabolism in patients with hemodialysis.MethodssEighty-six patients who were treated by hemodialysis in the First Affiliated Hospital of Xinxiang Medical University from August 2019 to August 2020 were selected,and they were divided into combination group(43 cases)and control group(43 cases)by random number table method.The control group was treated by hemodialysis alone,the combination group was treated with compoundα-ketoacids based on the treatment of the control group.The residual renal function,calcium-phosphorus metabolism,levels of inflammatory factors before and after treatment,and total adverse reactions were compared between the two groups.Results After treatment,the RRF of the two groups were lower than those before treatment(P<0.01),while the RRF of the combination group was higher than that of the control group(P<0.01);after treatment,the levels of blood calcium,blood phosphorus,calcium phosphorus product,and parathyroid hormone in the combination group were lower than those before treatment(P<0.01),while the levels of blood calcium,blood phosphorus,calcium phosphorus product,and parathyroid hormone in the combination group were lower than those in the control group(P<0.01).After treatment,the levels of interleukin-6,tumor necrosis factor-αand hypersensitivity C-reactive protein in the combination group were lower than those before treatment(P<0.01),while the levels of interleukin-6,tumor necrosis factor-αand hypersensitivity C-reactive protein in the combination group were lower than those in the control group(P<0.01);after treatment,there was no statistically significant difference in the total incidence of adverse reactions between the combination group(6.98%,3/43)and the control group(9.30%,4/43),X=0.16,P>0.05.Conclusions Compoundα-ketoacids can significantly improve the RRF,calcium-phosphorus metabolism and inflammatory response in patients with hemodialysis,without serious
