摘 要:目的:探讨不同浓度碳酸氢钠漱口预防慢性阻塞性肺疾病(COPD)患者长期激素雾化治疗所致口腔及下呼吸道真菌感染的效果。方法:抽取2019年2月至2022年1月联勤保障部队第九八九医院收治的COPD患者102例,拟行激素雾化治疗,三餐后予以不同浓度(2%、3%、4%)碳酸氢钠含漱,按照碳酸氢钠浓度将其分为低浓度组、中浓度组及高浓度组,每组34例。比较不同浓度组口腔、下呼吸道真菌感染发生率、感染发生时间、治愈时间,分析不同时间(漱口3、6、10 d)不同浓度组pH值、口腔清洁度、漱口依从性及痰培养结果。结果:高浓度组口腔、下呼吸道真菌感染发生率低于低浓度组及中浓度组(P<0.05);口腔、下呼吸道感染发生时间比较,高浓度组>中浓度组>低浓度组(P<0.05);治愈时间比较,高浓度组<中浓度组<低浓度组(P<0.05)。漱口3、6、10 d,中高浓度组口腔真菌感染患者pH值高于低浓度组口腔真菌感染患者(P<0.05);漱口6、10 d,中高浓度组口腔真菌感染患者口腔清洁度高于低浓度组口腔真菌感染患者(P<0.05)。中高浓度组口腔真菌感染患者漱口依从性[(3.18±0.31)分]高于低浓度组口腔真菌感染患者[(2.93±0.26)分],P<0.05。痰培养结果中,中高浓度组各类真菌占比低于低浓度组口腔真菌感染患者。结论:与2%、3%碳酸氢钠比较,4%碳酸氢钠漱口对COPD长期雾化激素治疗所致口腔、下呼吸道真菌感染效果显著,可降低真菌感染发生率,提高口腔pH值、清洁度及漱口依从性。Objective To investigate the effect of gargling with different concentrations of sodium bicarbonate on preventing oral and lower respiratory tract fungal infections caused by long-term aerosol steroid therapy in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 102 patients with COPD who planned to receive aerosol hormone therapy in the No.989 Hospital of the Joint Logistic Support Force from February 2019 to January 2022 were selected.The patients were given different concentrations(2%,3%,4%)of sodium bicarbonate to gargle after all meals,and they were divided into low concentration group,medium concentration group and high concentration group according to the concentration of sodium bicarbonate,with 34 cases in each group.The incidence of oral and lower respiratory tract fungal infections,the time of infection,and the time of cure of the three groups with dfferent concentrations were compared.The potential hydrogen(pH)value,oral cleanliness,mouthwash compliance and sputum culture results of the three groups with different concentrations were compared at different times(gargling for 3,6,and 10 days).Results The incidence of oral and lower respiratory fungal infections in the high concentration group were lower than those in the low concentration group and the middle concentration group(P<0.05).The occurrence time of oral and lower respiratory tract infection in the high concentration group were longer,followed by medium concentration group,low concentration group,in sequence(P<0.05);the healing time had a growing trend in the order of high concentration group,medium concentration group,low concentration group(P<0.05).Affer gargling for 3,6,and 10 days,the pH value of the patients with oralfungal infections of the middle concentration group and high concentration group were higher than that of the low concentration group(P<0.05);after gargling for 6 and 10 days,the oral cleanliness of the patients with oralfungal infections of the medium concentration group and high concentration group
