摘 要:目的 观察益原素茶晶对糖耐量及胰岛素分泌的影响,分析其对糖代谢影响可能的机制。方法 将新诊断为糖尿病前期及新发2型糖尿病患者随机分为对照组和试验组。以患者自身为对照,对照组患者食用馒头餐100 g同时饮用水,试验组食用馒头餐100 g同时饮用茶晶2 g,测定空腹、食用馒头餐后0.5、1.0、2.0和3.0 h的血糖、胰岛素、C肽、胰高血糖素及胰高血糖素样肽(GLP-1)。结果 入组125例,脱落25例,最后100例受试者纳入统计分析。试验组餐后1和2 h胰岛素水平、胰岛素曲线下面积分别为(67.25±35.07)μU·mL^(-1)、(73.66±51.44)μU·mL^(-1)、(194.40±107.10)μU·h·mL^(-1),对照组1和2 h的胰岛素、胰岛素曲线下面积分别为(59.87±31.42)μU·mL^(-1)、(64.96±43.62)μU·mL^(-1)和(183.17±100.10)μU·h·mL^(-1);试验组和对照组3 h胰高血糖素分别为(13.93±8.28)和(15.34±12.14) pg·mL^(-1);试验组1和2 h GLP-1、GLP-1曲线下面积分别为(1.61±1.37) pmol·L^(-1)、(1.57±1.35) pmol·L^(-1)和(6.29±5.32) pmol·h·L^(-1),对照组1和2 h GLP-1、GLP-1曲线下面积分别为(1.45±0.98)pmol·L^(-1)、(1.43±1.00)pmol·L^(-1)和(5.80±3.87)pmol·h·L^(-1)。试验组与对照组比较,1和2 h胰岛素及胰岛素曲线下面积、3 h胰高血糖素、1和2 h GLP-1及GLP-1曲线下面积差异均有统计学意义(均P<0.05)。结论 益原素茶晶可能通过增加餐后胰岛β细胞胰岛素的分泌,减少糖耐量受损者餐后胰高血糖素的分泌,促进其餐后GLP-1的分泌。Objective To explore the effect of ELITEA essence on glucose tolerance and insulin secretion,and analyze the possible mechanism on the glucose metabolism.Methods Patients newly diagnosed with prediabetes or newly onset type 2 diabetes were randomly divided into control group and treatment group.Using the patient’s own body as a control,the control group consumed 100 g of steamed bread and water,while the treatment group consumed 100 g of steamed bread and 2 g of ELITEA essence.The levels of blood glucose,insulin,C-peptide,glucagon-like peptide-1(GLP-1)at fasting,0.5 h,1 h,2h,3 h were detected.Results A total of 125 subjects were included in the trial,with 25 dropouts,resulting in 100 subjects ultimately included in the statistical analysis.In the treatment group,the insulin at 1 h and 2h and insulin area under the curve(AUC)were(67.25±35.07)μU·mL^(-1),(73.66±51.44)μU·mL^(-1)and(194.40±107.10)μU·h·mL^(-1),respectively.In the control group,the insulin at 1 h and 2 h and insulin AUC were(59.87±31.42)μU·mL^(-1),(64.96±43.62)μU·mL^(-1)and(183.17±100.10)μU·h·mL^(-1).The glucagon levels of the treatment group and the control group at 3 h were(13.93±8.28)and(15.34±12.14)pg·mL^(-1).The GLP-1 at 1 h and 2 h and GLP-1 AUC in treatment group were(1.61±1.37)pmol·L^(-1),(1.57±1.35)pmol·L^(-1)and(6.29±5.32)pmol·h·L^(-1).In the control group,the GLP-1 at 1 h and 2 h and GLP-1 AUC were(1.45±0.98)pmol·L^(-1),(1.43±1.00)pmol·h·L^(-1)and(5.80±3.87)pmol·L^(-1).Compared with the control group,there were significant differences in insulin at 1 h,2 h and AUC,glucagon at 3 h,GLP-1 at 1 h,2 h,AUC in the treatment group(all P<0.05).Conclusion ELITEA essence maybe contributed to pancreaticβcells to secrete insulin after meals,it can reduce the secretion of glucagon after meals and promote the secretion of postprandial GLP-1 in patients with impaired glucose tolerance.
