摘　　要：目的探究医疗失效模式与效应分析(HFMEA)在手术室常规病理标本分析前阶段管理中的应用效果。方法确立主题,组建HFMEA项目管理小组,绘制工作流程图,确定手术室常规病理标本在分析前阶段管理的失效模式,分析并确认根本原因,拟定行动计划并组织实施,进行效果评价。结果改进后,分析前阶段标本信息准确率和标本质量合格率得到明显改善(P<0.05)。结论应用HFMEA对手术室常规病理标本进行分析前阶段管理,可以有效、安全地管理手术室常规病理标本,提高手术室的护理质量。Objective To investigate the effectiveness of healthcare failure mode and effect analysis(HFMEA)in the management of the pre-analytical phase of routine pathology specimens in the operating room.Methods To establish the theme,form the HFMEA project management team,draw the workflow diagram,determine the failure mode of routine pathology specimen management in the pre-analytical stage of the operating room,analyze and confirm the root cause,formulate the action plan and organize the implementation,and evaluate the effect.Results After the improvement,the accuracy of specimen information and specimen quality pass rate in the pre-analytical stage were significantly improved(P<0.05).Conclusion Applying HFMEA to manage routine pathology specimens in the operating room at the pre-analytical stage can effectively and safely manage routine pathology specimens in the operating room and improve the quality of care in the operating room.
