摘 要:目的探讨急性淋巴细胞白血病患者微小RNA-377(miR-377)和SET及MYND结构域包含蛋白3(SMYD3)表达水平及临床预后意义。方法选择2016年3月至2020年6月该院收治的135例急性淋巴细胞白血病患者作为观察组,同时选择同期120例体检健康者作为对照组。采用实时荧光定量聚合酶链反应(qRT-PCR)检测研究对象外周血miR-377、SMYD3表达水平。随后比较不同组别、不同临床特征及不同疗效的患者miR-377、SMYD3表达水平差异,并采用Kaplan-Meier法和多因素Cox回归模型分析miR-377、SMYD3与急性淋巴细胞白血病患者预后的关系。结果观察组miR-377表达水平低于对照组,SMYD3表达水平高于对照组(P<0.05)。miR-377高表达组患者的危险度分型,白细胞计数(WBC)(入院时、入院1月后),乳酸脱氢酶(LDH)(入院时、入院1月后),骨髓原幼细胞比例均明显低于miR-377低表达组(P<0.05)。SMYD3高表达组患者的危险度分型,WBC(入院时、入院1月后),LDH(入院时、入院1月后),骨髓原幼细胞比例均明显高于SMYD3低表达组(P<0.05)。完全缓解(CR)组miR-377、SMYD3表达水平与对照组比较差异均无统计学意义(P>0.05),miR-377表达水平从低到高依次为未缓解(NR)组、部分缓解(PR)组、CR组、对照组(P<0.05),SMYD3表达水平从高到低依次为NR组、PR组、CR组、对照组(P<0.05)。miR-377高表达组总生存率(65.33%)明显高于miR-377低表达组(41.67%)(P<0.05),SMYD3高表达组总生存率(41.94%)明显低于SMYD3低表达组(65.75%)(P<0.05)。多因素Cox回归模型分析显示,危险度分型、WBC、LDH、骨髓原幼细胞比例、miR-377、SMYD3均为影响急性淋巴细胞白血病患者预后的危险因素(P<0.05)。结论急性淋巴细胞白血病患者miR-377呈低表达水平,SMYD3呈高表达水平,且二者表达水平与患者临床特征、疗效及预后均密切相关,因此可作为评估患者临床预后的有效指标。Objective To explore the expression levels and clinical prognostic significance of microRNA-377(miR-377)and SET and MYND domain-containing protein 3(SMYD3)in patients with acute lymphoblastic leukemia.Methods A total of 135 cases with lymphocytic leukemia treated in the hospital were selected as the observation group,and 120 healthy subjects were selected as the control group.The expression levels of miR-377 and SMYD3 in peripheral blood of the subjects were detected by qRT-PCR.Then,the differences in the expression levels of miR-377 and SMYD3 among cases with different groups,clinical characteristics and different therapeutic effects were compared,and the Kaplan-Meier method and multivariate Cox regression model were used to analyze the relationship between miR-377 and SMYD3 and the prognosis of cases with acute lymphoblastic leukemia.Results The expression level of miR-377 in the observation group was lower compared with control group,and the expression level of SMYD3 was higher compared with the control group(P<0.05).The risk classification,white blood cell(WBC)(at admission,one month after admission),lactate dehydrogenase(LDH)(at admission,one month after admission),and the proportion of bone marrow blasts in the miR-377 high expression group were significantly lower than those in the miR-377 low expression group(P<0.05).The risk classification,WBC(at admission,one month after admission),LDH(at admission,one month after admission),and the proportion of bone marrow protoblasts in the SMYD3 high expression group were significantly higher than those in the SMYD3 low expression group(P<0.05).The expression levels of miR-377 and SMYD3 in the complete remission(CR)group were not significantly different from those in the control group(P>0.05).The expression levels of miR-377 in four groups from low to high were no response(NR)group,partial remission(PR)group,CR group and control group(P<0.05),and the expression level of SMYD3 in four groups from high to low were NR group,PR group,CR group and control group(P<0.05).T
