常规超声及超声造影在腮腺基底细胞腺瘤和沃辛瘤鉴别诊断中的价值

摘　　要：目的探讨常规超声及超声造影(CEUS)在腮腺基底细胞腺瘤(BCA)和沃辛瘤(WT)鉴别诊断中的价值。方法回顾性分析30个BCA肿块和34个WT肿块的超声及CEUS特征,对比分析两组间回声、血流、阻力指数(RI)、增强强度、增强模式、增强和消退方式等差异,手术病理结果为诊断的金标准。结果BCA组和WT组间肿块直径、血流分级、增强模式、增强方式及增强强度无明显差异(P>0.01),两组间男女比例、年龄、是否囊性变、RI、血流分级、有无增强环和消退方式的差异有统计学意义(P<0.05)。结论腮腺BCA和WT在临床和超声特征具有相似之处,但利用患者性别、年龄、是否囊性变、RI、血流分级、有无增强环和消退方式有助于两者的鉴别诊断。Objective To explore the value of conventional ultrasound and contrast-enhanced ultrasonography(CEUS)in the differential diagnosis of basal cell adenoma(BCA)and Warthin tumor(WT).Methods The ultrasound and CEUS features of 30 BCA masses and 34 WT masses were retrospectively analyzed,and the inter-group differences in echo-density,blood flow,resistance index(RI),enhanced intensity,enhanced mode,enhanced approach and receding approach were analyzed,and the surgical pathological results were used as the gold standard for diagnosis.Results There were no significant differences in mass diameter,blood flow classification,enhanced mode,enhanced approach and enhanced intensity between the BCA group and the WT group(P<0.01),and there were significant differences in male and female ratio,age,cystic degeneration,RI,blood flow classification,presence or absence of enhanced ring and receding approach between the two groups(P<0.05).Conclusions BCA and WT have similar clinical and ultrasound features,but gender,age,cystic degeneration,RI,blood flow classification,presence or absence of enhanced ring,and receding approach are helpful for the differential diagnosis of the two.
