成人心脏乳头状弹力纤维瘤超声心动图特征及漏诊误诊分析

摘　　要：目的探讨成人心脏乳头状弹力纤维瘤超声心动图特征。方法回顾性分析近5年经手术病理证实的26例成人心脏乳头状弹力纤维瘤患者资料,总结超声心动图特征。结果26例患者超声心动图显示21例,漏诊5例。乳头状弹力纤维瘤单发多见,好发于瓣膜(70.37%),以主动脉为主。病灶最大径平均(12.95±4.43)mm,超声心动图表现呈中等回声,质地疏松;呈类圆形(66.67%),边缘不光整,常可见成角或小叶,呈“绒球状”,7例呈不规则形;17例可见蒂(80.95%),4例未见明显蒂结构;18例可见较大活动度(85.71%),肿瘤活动时可见发生形变;瓣膜开放不受限。结论成人心脏乳头状弹力纤维瘤超声表现有一定特征性,好发于心脏瓣膜,合并其他瓣膜病或病灶太小时容易漏诊。Objective To explore the echocardiographic characteristics of adult cardiac papillary fibroelastoma.Methods The data of 26 adult patients with cardiac papillary elastofibroma confirmed by operation and pathology in Zhongshan Hospital of Fudan University in recent 5 years were retrospectively analyzed,and the echocardiographic features were summarized.Results Echocardiography of 26 patients showed 21 cases and missed 5 cases.The solitary papillary elastofibroma was more common in solitary,and mostly in the valve(70.37%),mainly in the aorta.The maximum diameter of the the lesion was(12.95±4.43)mm.Echocardiography showed moderate echo and loose texture.The shape was round(66.67%),the edge was not smooth,and the angle or lobule was seen,which was"pompom",and 7 cases were irregular.Pedicle was visible in 17 cases(80.95%),and no obvious pedicle structure was found in 4 cases.18 cases(85.71%)had large motion,and deformation was observed during the tumor was moving.The valve opening was not restricted.Conclusions Ultrasonic characteristics of cardiac papillary elastofibroma in adults have certain ultrasonic characteristics.It is more likely occurs in the heart valve,and it is easy to miss diagnosis when combined with other valvular diseases or too small lesions.
