胎儿硬脑膜窦扩张并血栓形成的超声表现分析

摘　　要：目的探讨胎儿硬脑膜窦扩张并血栓形成的二维及彩色多普勒超声声像图特点,加强超声医师对该疾病的认识,帮助临床评价胎儿预后。方法总结5例产前超声诊断并经MRI确诊的硬脑膜窦扩张并血栓形成的胎儿声像图特点,随访妊娠结局。结果5例胎儿超声均表现为窦汇及上矢状窦呈三角形扩张,内见血栓形成,血栓表现为不规则形、圆形或三角形,硬脑膜窦周边未见动-静脉分流。其中3例胎儿硬脑膜窦及血栓随着孕周的增大而缩小,出生后常规儿童保健未见异常,2例行颅脑MRI检查未见异常。5例胎儿中1例引产后未尸检,1例失访。结论胎儿硬脑膜窦扩张并血栓形成的超声表现为颅骨前方硬脑膜窦扩张呈三角形,内血栓呈不规则形、圆形或三角形,大小和回声随着孕周的增大而改变。连续的超声检查显示肿块缩小,甚至消失者预后良好。Objective To investigate the characteristics of two-dimensional and color Doppler ultrasonography of fetal dural sinus dilatation complicated with thrombosis,to strengthen the understanding of ultrasonographer on this disease and to help the clinicians with evaluation of the fetal prognosis.Methods The ultrasonographic features of five cases with dural sinus dilatation complicated with thrombosis confirmed by MRI were summarized,and the fetal pregnancy outcome was followed up.Results In all five fetuses,a triangular dilatation of the torcular herophili and superior sagittal sinus and thrombosis was found on ultrasound examination.The thrombosis appeared irregular,round,or triangular,with no arteriovenous shunt around dural sinuses.Among them,three cases of fetal dural sinus and thrombus decreased with the increase of gestational age,and no abnormalities were found in routine child care after birth.The other two cases showed no abnormalities on brain MRI examination.Among the five fetuses,one fetus was not autopsied after labor induction,and the other was lost to follow-up.Conclusions The ultrasonography of fetal dural sinus dilatation complicated with thrombosis showed that the dural sinus dilatation is triangular in front of skull,and the thrombus could be irregular,circular or triangular in shape.The size and echo changed with the increase of gestational age.Continuous ultrasound examination shows that those with reduced or even disappeared masses have a good prognosis.
