摘　　要：目的分析卵巢甲状腺肿的临床特征及超声表现。方法回顾性分析19例经手术治疗后病理证实为卵巢甲状腺肿患者的临床资料及超声表现,包括位置、大小、形态、边界、有无分隔、回声特点及血流信号等特征。结果19例患者共计19个卵巢甲状腺肿病灶,其中3例合并囊性成熟性畸胎瘤,3例伴滤泡上皮异型增生,1例为囊性成熟性畸胎瘤伴卵巢甲状腺肿性类癌。超声表现:附件区囊性包块4例、附件区囊实混合性包块(囊性为主)10例、附件区囊实混合性包块(实性为主)5例。结论卵巢甲状腺肿超声声像图常表现为单侧多房囊性或囊实混合性肿瘤,边界较清晰,伴有不规则增厚分隔。囊实混合者囊内可见结节状中等或高回声突起伴丰富血流信号,或类圆形“白球样”特征性高回声团,临床结合患者腹水体征及CA125、甲状腺球蛋白的升高可考虑卵巢甲状腺肿的可能。Objective To discuss the clinical features and sonographic findings of struma ovarii(SO).Methods The ultrasound manifestations and clinical data of 19 patients with SO confirmed by postoperative pathology after surgical treatment were retrospectively reviewed,including location,size,morphology,boundary,presence of separation,echo characteristics and blood flow signals.Results There were 19 lesions in 19 patients,3 cases were SO with ovarian mature cystic teratoma,3 cases with follicular epithelial dysplasia,and 1 case was ovarian mature cystic teratoma with strumal carcinoid tumor of the ovary(SCTO).Ultrasonic image indicated that 4 cases had cystic mass in adnexal area,10 had cystic-solid mass(mainly dominated by cyst),and 5 had cystic-solid mass(mainly dominated by solid).Conclusions Ultrasonography of SO generally manifested as unilateral multilocular cystic or cystic-solid mass with clear boundary and irregular thickening separation.Moderate or hyperechoic nodular solid protrusions with rich blood flow signals or characteristic hyperechoic mass as“white”can be detected in the cysic components of cystic-solid ones,combined with ascites and the elevation of CA125 and thyroglobulin(Tg),the possibility of SO could be considered.
