健康生态学视角下慢性心力衰竭患者健康管理模式的构建

摘　　要：目的构建科学、长效的社区慢性心力衰竭健康患者管理模式。方法通过文献法和小组讨论,基于健康生态学模型,形成专家函询问卷;采用德尔菲法,选取江苏省13所三甲医院和社区医院的43位专家进行2轮函询。结果2轮问卷有效回收率分别为100%、95.6%,专家权威系数分别为0.88、0.89,和谐系数均有统计学意义(P<0.001)。构建了基于健康生态学的慢性心力衰竭健康管理模式,包含3个一级条目、18个二级条目、96个三级条目。结论本研究构建的慢性心力衰竭健康管理模式的科学性和实用性较好,为社区慢性心力衰竭患者的健康管理提供了良好借鉴。Objective To establish a scientific and long-term community health management model for patients with chronic heart failure.Methods Based on the health ecology model,the expert consultation questionnaire was formed through literature review and experts discussion;Using Delphi method,43 experts from 13 Grade A hospitals and community hospitals in Jiangsu province were selected for 2 rounds of letter consultation.Results The effective return rates of the 2 rounds questionnaires was 100%and 95.6%,respectively,and the experts'authority coefficient was 0.88 and 0.89,respectively.The harmony coefficient of the 2 rounds of expert consultation was statistically significant(P<0.001).A health management model for chronic heart failure based on health ecology was constructed,including 3 first-level items,18 second-level items,and 96 third-level items.Conclusion The health management model of chronic heart failure constructed in this study is scientific and practical,and provides a good reference for the community health management of patients with chronic heart failure.
