疼痛管理干预对晚期癌痛患者睡眠质量及心理状况的影响分析

摘　　要：目的:探讨采用护士为主,医师、麻醉师为辅(NBAS-APS)模式下的疼痛管理干预对晚期癌痛患者睡眠质量及心理状况的影响。方法:选取2020年7月至2022年7月复旦大学附属中山医院厦门医院收治的晚期癌痛患者86例作为研究对象,按照随机数字表法随机分为观察组和对照组,每组43例。观察组采用NBAS-APS模式下的疼痛管理干预,对照组采用常规护理方法。比较2组患者干预前后的疼痛情况[癌症疼痛障碍问卷(BQ)、视觉模拟评分法(VAS)、疼痛评估数字等级评定量表(NRS)]、睡眠质量[匹兹堡睡眠质量指数量表(PSQI)]、心理状况[汉密尔顿焦虑自评量表(HAMA)、汉密尔顿抑郁自评量表(HAMD)]的变化。结果:干预后,2组BQ、VAS、NRS评分水平均降低,且观察组均低于对照组(P<0.05);干预后,2组PSQI、HAMA、HAMD评分均下降,且观察组低于对照组(P<0.05)。结论:NBAS-APS模式下的疼痛管理干预可有效提高晚期癌痛患者睡眠质量,改善其心理状况。Objective:To explore the impact of pain management intervention using the NBAS-APS model,where nurses are the main focus and physicians and anesthesiologists are the auxiliary,on the sleep quality and psychological status of patients with advanced cancer pain.Methods:A study was conducted on 86 patients with advanced cancer pain admitted to Fudan University Affiliated Zhongshan Hospital Xiamen Hospital from July 2020 to July 2022.Patients with usual care were divided into control group(43 patients),and patients with pain management intervention under the NBAS-APS model were divided into observation group(43 patients).Compare the changes in pain status(BQ,VAS,NRS),sleep quality(PSQI),psychological status(HAMA,HAMD),and nursing satisfaction before and after intervention between two groups of patients.Results:After intervention,the levels of BQ score,VAS score,and NRS score in both groups decreased,and the observation group was lower than the control group(P<0.05);After intervention,the PSQI scores of both groups decreased,and the observation group was lower than the control group(P<0.05);After intervention,the HAMA score and HAMD score of both groups decreased,and the observation group was lower than the control group(P<0.05).Conclusion:The pain management intervention under the NBAS-APS mode can effectively improve the sleep quality of patients with advanced cancer pain,improve their psychological status.
