睡眠呼吸暂停合并肺心病呼吸衰竭中应用持续气道正压通气治疗的效果分析

摘　　要：目的:分析睡眠呼吸暂停合并肺心病呼吸衰竭中应用持续气道正压通气治疗的效果。方法:选取2020年6月至2022年3月山东省菏泽市郓城县中医医院内二科收治的睡眠呼吸暂停合并肺心病呼吸衰竭患者86例作为研究对象,按照随机数字表法随机分为对照组和观察组,每组43例。对照组实施常规治疗手段,观察组增加持续气道正压通气治疗,观察治疗前后2组的血气指标、睡眠呼吸参数和生命质量。结果:治疗后,观察组的MAT、AHI以及最低SaO_(2)、最高SaO_(2)改善情况较对照组更优,观察组SaO_(2)、PaO_(2)以及PaCO_(2)等指标水平改善情况较对照组更优,观察组的生命质量评分较对照组高,差异有统计学意义(P<0.05)。结论:睡眠呼吸暂停合并肺心病呼吸衰竭应用持续气道正压通气治疗能显著改善患者的睡眠和呼吸水平,同时能有效调节血气指标,进而帮助患者提高整体治疗效果和生命质量。Objective:To analyze the effect of continuous positive airway pressure(CPAP)on sleep apnea with Respiratory failure caused by cor pulmonale.Methods:A total of 86 patients with sleep apnea combined with Respiratory failure of cor pulmonale who were hospitalized in Yuncheng County hospital of Traditional Chinese medicine from June 2020 to March 2022 were selected and randomly divided into control group and observation group by digital random method,each group has 43 cases.The control group received routine treatment,while the research group received continuous positive airway pressure ventilation treatment.Blood gas indicators,sleep breathing parameters,and quality of life were observed before and after treatment in both groups.Results:Compared with the control group,the observation group showed better improvement in MAT,AHI,and the lowest and highest SaO_(2) levels after treatment,with statistically significant data(P<0.05).Compared with the control group,the improvement of indicators such as SaO_(2),PaO_(2),and PaCO_(2) levels in the observation group was better after treatment,with statistically significant data(P<0.05).The quality of life score of the observation group patients after treatment was higher than that of the control group,and the data was statistically significant,P<0.05.Conclusion:Continuous positive airway pressure ventilation can significantly improve the sleep and breathing level of patients with sleep apnea and Respiratory failure caused by cor pulmonale,and can effectively regulate blood gas indicators,thereby helping patients improve the overall treatment effect and quality of life.
