摘 要:目的探讨氨茶碱联合百令胶囊对稳定期慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者气道功能及血清气道重塑分子的影响。方法选取96例稳定期COPD患者,用随机数字表法分为3组(对照组、A组和B组),每组32例。对照组接受常规西药(硫酸沙丁胺醇吸入气雾剂联合布地奈德福莫特罗吸入粉雾剂)治疗,在对照组治疗的基础上,A组加用氨茶碱、B组加用氨茶碱和百令胶囊。观察3组治疗前后的气道功能参数、血清炎症因子、生长因子、气道重塑分子变化及生活质量评分,对比治疗效果及药物不良反应发生情况。结果治疗后,3组的第1秒用力呼气容积(forced expiratory volume in 1 second,FEV 1)、第1秒用力呼气容积与用力肺活量比值(forced expiratory volume in 1 second/forced vital capacity,FEV 1/FVC)、50%肺活量最大呼气流速(maximal expiratory flow after 50%of the FVC has been not exhaled,MEF 50)、中段呼气流速(maximal expiratory flow after 25%~75%of the FVC has been not exhaled,MEF 25~75)和25%肺活量用力呼气流速(maximal expiratory flow after 25%of the FVC has been not exhaled,MEF 25)均明显升高(P<0.05),3组治疗后的各气道功能参数比较差异有统计学意义,FEV 1、FEV 1/FVC、MEF 50、MEF 25~75和MEF 25均为B组>A组>对照组,P<0.05。3组治疗后血清白细胞介素2(interleukin-2,IL-2)、白细胞介素-18(interleukin-18,IL-18)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、神经生长因子(nerve growth factor,NGF)及碱性成纤维细胞生长因子(basic fibroblast growth factor,b-FGF)水平均明显降低(P<0.05)。3组比较差异有统计学意义,且均为B组<A组<对照组(P<0.05)。3组的转化生长因子β1(transforming growth factor-β1,TGF-β1)、烟酰胺腺嘌呤二核苷酸磷酸氧化酶4(nicotinamide adenine dinucleotide phosphate oxidase 4,NOX4)、核因子-κB(nuclear factor-κB,NF-κB)及基质金属蛋白酶9(matrix metalloproteinase-9,MMP-9)水平Objective To investigate the effects of aminophylline combined with Bailing Capsules on airway function and serum airway remodeling molecules in patients with stable chronic obstructive pulmonary disease(COPD).Methods 96 patients with stable COPD were randomly divided into 3 groups,with 32 cases in each group.The control group received conventional western medicine(Salbutamol Sulfate Inhalation Aerosol and Budesonide Formoterol Inhalation Powder Aerosol),the group A was added with aminophylline,and the group B was added with aminophylline and Bailing Capsules.The changes of airway function parameters,serum inflammatory factors,growth factors,molecular level of airway remodeling and quality of life scores of the 3 groups were observed before and after treatment,and the treatment effects and adverse drug reactions were compared.Results Compared with before treatment,the levels of forced expiratory volume in 1 second(FEV 1),forced expiratory volume in 1 second/forced vital capacity(FEV 1/FVC),maximal expiratory flow after 50%of the FVC has been not exhaled(MEF 50),maximal expiratory flow after 25%-75%of the FVC has been not exhaled(MEF 25-75)and maximal expiratory flow after 25%of the FVC has been not exhaled(MEF 25)in the 3 groups were significantly increased after treatment(P<0.05).There was significant difference in the airway function parameters of the 3 groups after treatment(P<0.05).The level trend was group B>group A>control group(P<0.05).Compared with before treatment,the serum levels of interleukin-2(IL-2),interleukin-18(IL-18),vascular endothelial growth factor(VEGF),nerve growth factor(NGF)and basic fibroblast growth factor(b-FGF)in the 3 groups after treatment were significantly reduced(P<0.05).The level trend was group B<group A<control group(P<0.05).Compared with before treatment,the levels of transforming growth factor-β1(TGF-β1),nicotinamide adenine dinucleotide phosphate oxidase 4(NOX4),nuclear factor-κB(NF-κB)and matrix metalloproteinase9(MMP9)after treatment in the 3 groups decreased significa
